Study On The Status Quo Of Continuous Nursing Needs And Intervention Effect Of Community Gout Patients

Objectives:To compile a questionnaire for continuous nursing needs for patients with gout in the community,carry out a scientific evaluation of the questionnaire,carry out continuous nursing and evaluate the effectiveness of the continuous nursing.Methods:1.The simple random sampling method was used to extract gout patients in the community,through literature analysis,the continuity of care group discussion and expert consultation to form "items and dimensions of community patients with gout needs continuing nursing care questionnaire",the reliability and validity of the questionnaire for scientific evaluation.2.A questionnaire survey was conducted on community gout patients by using the basic data questionnaire of community gout patients and the self-designed questionnaire of continuous nursing needs of community gout patients.Analysis of scores of different gender of extended care needs of the impact of the use of two sample t test scores on the continuing nursing care questionnaire total score and scores of each dimension of the gout patients by using univariate analysis of variance of different cultural degree,average family income,occupation,marriage,living,medical payment,duration,complications;questionnaire the total score and life care demand dimensions,psychological care needs dimension,psychological care needs continuous care needs dimension,disease prevention needs of self care the scores as the dependent variable and influencing factors by stepwise analysis of continuity care needs of the community in patients with gout,including alpha in=0.05,alpha out =0.10.3.By objective sampling method,according to the results of the questionnaire and the influence factors of demand for voluntary principles into 100 community gout patients,divided into control group and experimental group,50 patients in each group,the experimental group according to the demand for the continuity of the questionnaire results of the nursing measures,the control group with general nursing service to participate in community activities,the intervention time was 3 months.The patients with basic information questionnaire and blood biochemical indexes,the continuity of care evaluation scale,gout self-management questionnaire will be compared.The data were recorded in SPSS20.0 for statistical analysis.Results:1.The dimensions and the reliability and retest reliability of the continuous care needs questionnaire for community gout patients were all greater than 0.6,and the variance explained by 6 principal factors accounted for 76.275% in exploratory factor analysis.2.Community patients with gout needs continuing nursing care questionnaire total score was 56.152 ± 5.703,demand,life care and rehabilitation care needs,psychological care needs,continue nursing needs,disease prevention and self care needs demand the scores were 6.929±0.701,11.804±1.341,7.907±0.921,8.853±1.550,10.798±1.304,9.815 ±1.288.The male patients,qualifications for college and above,the average monthly household income is 2000-3000 yuan,occupation for staff,marital status,married living with spouse and children living,medical fees,payment for the duration of extended care needs higher 1-5 years,gout patients complicated with diabetes mellitus.Continuity of care needs of the community of gout patients is affected by many factors,including gender,education level,family monthly income,occupation,medical payment,duration and complications were the factors influencing total continuity of demand;education degree,average family income,medical payment,complications are the influence factors of life care demand;family average monthly income,medical payment,course of disease,complications are the factors influencing rehabilitation nursing demand;gender,occupation,complications are the influence factors of psychological nursing demand;education degree,average family income,medical payment,duration and complications are the factors influencing continuous care needs;and gender,education level,medical payment,disease is the factors affecting disease prevention needs;education level,marital status,medical payment Type and complication are the factors affecting the needs of self care.3.To evaluate the effect of community nursing for the continuation of 3 gout patients and the control group,UA,Cho,TG index,GLU,ALT of the intervention group was 0.045,0.006,0.044,0.032,0.042,P index is less than 0.05,the difference was statistically significant,the indexes of Cr,BUN value of the P score 0.248,0.40,P > 0.05 there were no significant differences;the continuity of nursing intervention in continuing nursing care evaluation scale and gout self-management scale in each dimension and the total scale score of experimental group compared with the control group the difference was statistically significant(P < 0.05);the inclusion of the community patients with gout for the continuity of care sources of demand analysis,about 25% of the patients choose a healthy way about the continuity of care,and through the telephone for continuing nursing care accounted for only 2%,this means less.Conclusions:1 The reliability and validity of the "community gout patient's continuing care demand questionnaire" was established.2 The preparation of the "community of gout patients needs continuing nursing care of patients with gout questionnaire,community of situation and influencing factors of continuous care needs,show the gout patients community has extended care needs and have different factors affect the implementation of continuing care.3 Through the implementation of continuing nursing care measures,improve the biochemical indicators of patients,the continuation of evaluation and gouty nursing ability of self management has also been improved,in order to achieve the perfect nursing care requirements,standardized continuing nursing measures,in order to play the advantages of continuity of care,improve the quality of life of patients,reduce the burden of disease in patients with gout.
