Research On The Mode Of Combining Medical Care With Old-age Care In Urban Communities In China

With the deepening of the aging of the population,the problem of getting old before getting rich has gradually become prominent,and Chinese society will face the increasing pressure of old-age care.The aging of population has become a major challenge to China's social and economic development in the current stage and for a long time to come.By the end of 2017,the population aged 60 and above accounted for 17.3 percent of China's total population,with a total population of 241 million,according to the national aging office.And in the 13 th five-year plan period,the aging degree of China's population will continue to deepen.The report of the 19 th national congress of the communist party of China clearly puts forward that we should actively respond to the aging of the population,establish a policy system and social environment for providing for the aged,showing filial piety and respect for the aged,promote the integration of medical care and old-age care,and accelerate the development of undertakings and industries for the aged.Therefore,it will be a favorable choice to develop the mode of integrated community medical care and old-age care service in the face of severe old-age care challenges at this stage.From the perspective of the needs of the elderly,the traditional pension model can only meet the needs of its "support" module.For the demand of professional and continuous medical services,most of the elderly will choose to be hospitalized regularly.This will inevitably result in the waste of medical resources caused by the elderly crowd occupying beds for a long time.It is under the imbalance of supply and demand of the old-age care resources that the mode of community-based medical care combined with old-age care emerges.This mode breaks the previous independence of the two resources,effectively integrates the endowment and medical resources required by the elderly service,and provides diversified medical and nursing services for the people in need within the jurisdiction.This paper is divided into five parts.The first part is the introduction,which summarizes the research background and significance of this paper.The second part introduces the theoretical basis and the definition of related concepts.The third part has sorted out the domestic community medical and nursing related practices,and summarized and analyzed the problems and causes in the operation process.First of all,on the basis of the present stage our country city community medical combining with construction practice,it is concluded that four current operating mode,are: the community endowment institutions and community health service station cooperation pattern,community health service center with medical ward mode,community endowment service agencies to set up the medical support combined with medical services,hospital to undertake community support service agencies.Then,the existing problems in the service mode of combining community medical care with old-age care are summarized as follows: regulatory attribution,service content,practitioner qualification and financing channels,etc.,and the causes are analyzed.The fourth part,on the basis of the foregoing,draws on the advanced experience of developed countries in the world to extract the methods that can be applied in the field of the combination of community medicine and nursing in China at the present stage.Finally,the corresponding improvement countermeasures and Suggestions are put forward.The roles and responsibilities of the government,society and the public in the construction of the model of integrated old-age care in urban communities are clearly defined.It is suggested to make full use of the service resources of all parties for the elderly to establish a cross-regional,cross-departmental and cross-level collaborative system of integrated old-age care services in the community,so as to truly achieve the goal of "supporting the elderly and caring for the sick".
