Objective To observe the clinical effect of acupuncture combined with Scapula Proprioceptive Neuromuscular Facilitation(PNF)Technique on acute scapulohumeral periarthritis. Methods Selecting 66 patients of acute scapulohumeral periarthritis, who reach the research standard.Patients are randomly divided into three groups: acupuncture group,PNF technique group,acupuncture-PNF technique group,22 patients in each group.Beside the basic treatment, the acupuncture group was treated with acupuncture, acupoints such as Jianyu,Jianliao,Jianqian,Jianzhen,Quchi,Yanglingquan, and so on.on the injured side were selected. After conventional acupuncture and Deqi, the lifting and thrusting manipulation was performed, and the needling was performed every 5 mins, then retaining the needle for 20 mins.the PNF technique group was treated with PNF technique,it includes two modes: the shoulder blade forward lifting and backward pressing, and the shoulder blade forward pressing and backward lifting. The acupuncture-PNF technology group was treated with acupuncture combined with scapular PNF technology, and the acupuncture method was the same as the acupuncture group, and the scapular PNF technology was the same as the PNF technology group. The treatment of three groups were once a day, three times a week, all groups continuously treated for 4 weeks. The clinical efficacy criteria, visual analogue scale(VAS)and shoulder function scores(pain, shoulder mobility, muscle strength, daily living ability, joint local morphology)were used as observation indicators to observe after treatment the changes of three groups and the changes between groups. Results 1.The total effective rate of the acupuncture group after treatment(85.00%),the total effective rate of the PNF technique group(81.82%),and the total effective rate of the acupuncture-PNF technique group(95.24%).To compared the total effective rate of the three groups, and the difference was statistically significant.(P<0.05)2.VAS scores: the three groups were significantly lower than before treatment, the difference was statistically significant(P<0.05);After treatment compare groups in pairs : there was no significant difference between the acupuncture group and the acupuncture-PNF technique group(P<0.05).The VAS scores of the acupuncture group and the acupuncture-PNF technique group were lower than those of the PNF technique group, and the difference was statistically significant.(P<0.05)3.Shoulder joint function score: After treatment, the three groups had significant improvement in pain, shoulder joint activity and daily living ability(P<0.05),and there was no significant change in muscle strength and joint local morphology(P<0.05).Pairwise comparison of the three groups after treatment: in terms of improving pain, there was no significant difference between the acupuncture-PNF technique group and the acupuncture group(P<0.05).The acupuncture group and the acupuncture-PNF technique group were better than the PNF technique group. The difference was statistically significant(P<0.05);There was no significant difference between the acupuncture-PNF technique group and the PNF technique group in improving the mobility of the shoulder joint and daily living ability(P<0.05).The acupuncture-PNF technique group and the PNF technique group were superior to the acupuncture group. The difference was statistically significant(P<0.05).There was no significant difference between the three groups in improving muscle strength and local morphology of the joint(P<0.05).In terms of total scores, the three groups were significantly improved before treatment, and the differences were statistically significant(P<0.05).Pairwise comparison of the three groups after treatment: the acupuncture-PNF technique group was higher than the acupuncture group and the PNF technique group. The difference was statistically significant(P<0.05).There was no significant difference between the acupuncture group and the PNF technique group(P<0.05).Conclusion Acupuncture,PNF technique and acupuncture combined with PNF technique therapy can improve the clinical symptoms of patients with acute scapulohumeral periarthritis, and the three have different effects on symptom improvement.Acupuncture has advantages in improving pain.It is not effective in improving shoulder mobility and daily living ability.PNF technique therapy can improve shoulder mobility and daily living ability, and it is inferior to improving pain.Acupuncture and PNF technique therapy can not only improve pain well.It can also significantly improve the functional indexes such as shoulder mobility and daily living ability.Acupuncture combined with PNF technique therapy has a good clinical effect on acute scapulohumeral periarthritis. However, the improvement of muscle strength and joint local morphology is not obvious.Further research and verification is needed in the future.
