| Purpose:The medical industry is a special industry,which is closely related to human’s health and life safety.In recent years,under the function of market economy,some operators do not hesitate to violate the law in order to pursue the maximization of interests,so that the types and forms of illegal acts of medical institutions are diversified.Therefore,the supervision,inspection and punishment of medical institutions can standardize the diagnosis and treatment behavior of medical institutions and their employees,and play a positive role in purifying the medical market environment and ensuring the order of medical and health services.According to the above situation,the following study has been done in this thesis.This study mainly investigated the situation of health supervision in a certain district of Beijing from 2015 to 2017 to understand the basic situation of medical institutions in this area,the practice of medical institutions in accordance with the law and the situation of health administrative punishment and to find out the problems existing in medical institutions and medical related personnel in medical activities and the shortcomings and defects in the process of health supervision and inspection.The purpose of this thesis is to provide a scientific basis for creating a safe medical environment for the people,improving the supervision and law enforcement level of health supervisors,and providing reference for the government to revise laws and regulations and formulate health administrative decisions.Method:In this study,medical institutions engaged in medical and related activities in a district of Beijing from 2015 to 2017 were selected.Include 35 level I and above hospitals,115 clinics,outpatient departments and health clinics,and 433 clinics,service stations and infirmaries.By collecting the supervision records,punishment records and punishment files and documents of all medical institutions in the area,the data were collected and then,collated and summarized.The results of the investigation were statistically processed by Chi-square test in SPSS18.0.Results:1.The basic situation of medical institutions in a certain district of Beijing: there are 583 medical institutions in the district,and the medical service room of the health service station has the highest proportion,which is 74.3%..2.The supervision coverage rate of medical institutions was 92.39% in 2015 and 100% in 2016 and 2017.With the improvement of supervision coverage,the qualified rate of supervision showed a downward trend.3.Supervision of the qualifications of medical institutions and medical personnel in 2015-2017: the quality of supervision and inspection of medical institutions was higher,the qualification rate was statistically significant in different levels of medical institutions,and the medical institutions at the clinics were not qualified,which was the department with the highest unqualified rate.Licensed doctors and licensed nurses have less supervision and examination households,and the qualified rate distribution of different medical institutions was similar.4.Supervision of practice behavior of medical institutions from 2015 to 2017: in the past three years,the number of unqualified households in prescription management and medical advertising in medical institutions was only 4,with a high qualified rate.The qualified rate distribution of prescription management in different levels of medical institutions was statistically significant,and the unqualified rate of clinic out-patient department was the highest.5.Supervision of infectious diseases and disinfection in medical institutions in a certain district of Beijing from 2015 to 2017: the main inspection of infectious diseases and disinfection in accordance with the law includes prevention and control of infectious diseases and reporting of epidemic situation,pre-examination and triage of infectious diseases,implementation of isolation of hospital infection and disinfection,management of medical waste.Among them,the unqualified rate of the regulation of medical waste is higher.From 2015 to 2017,the implementation of hospital infection and disinfection isolation and the unqualified rate of medical waste management in clinic out-patient departments were the highest.6.From 2015 to 2017,there were 129 illegal cases in medical institutions,with an average of 43 penalties per year.The number of cases investigated and dealt with showed an increasing trend,of which the maximum number of cases investigated and dealt with in 2017 was 83,and the number of cases punished in 2015 was the lowest,with the number of 19.The illegal proportion of for-profit and non-profit(non-governmental)medical institutions,the illegal proportion of clinics and village clinics was high,and the phenomenon of employing non-health technical personnel in the health care department of the clinic was more prominent.In terms of infectious diseases and disinfection,there were the most problems in medical institutions at the village clinic level,and the problem of medical waste not being dealt with in accordance with the regulations was more prominent.In the period 2015-2017,the number of summary procedure cases was decreasing,while the number of general procedure cases was on the rise;In the administrative punishment of infectious diseases and disinfection,the number of summary procedure cases is increasing,and the increase is more obvious.The main types of punishment were warning and fining.The source of the case mainly comes from social reports and health supervision and inspection.During this period,there were 20 cases of unlicensed medical practice in a certain district of Beijing,and the number increased year by year,with the most cases in stomatology.Conclusion:1.Among the medical institutions in a certain district of Beijing,the proportion of the infirmary is the highest,and the nature is mainly non-profit(non-governmental)medical institutions.2.From 2015 to 2017,the qualified rate of health supervision of medical institutions in a certain district of Beijing showed a downward trend with the improvement of supervision coverage.3.From 2015 to 2017,the unqualified medical institutions in the district were mainly concentrated in the clinic,out-of-patient department and health care centers.4.The unqualified rate of the hospital infection,disinfection isolation and medical waste management was the highest at the level of clinic.The unqualified rate of medical waste management was the highest in 2017.5.The number of administrative punishment cases in medical institutions was increasing,among which there were more for-profit and non-profit(non-governmental)medical institutions,mainly concentrated in the medical institutions at the clinic and village clinic level.The medical institutions at the level of infirmary of village clinic service station have the highest proportion of punishment for failing to dispose of medical waste in accordance with the regulations.6.In 2015,the proportion of general punishment procedures was the highest,and the proportion of summary procedures in 2017 was the highest.Among the medical and administrative penalties,the proportion of fines in 2015 and 2016 was the highest,while the proportion of drug devices and illegal gains was the highest in 2017.Among the cases of infectious diseases and disinfection administrative punishments,the highest proportion of fines in 2015,and the highest proportion of warnings in 20177.The cases of unlicensed medical practice were mainly concentrated in non-medical institutions and individual medical practice,mainly in stomatology,followed by internal medicine,and the source of punishment cases was changed from complaint reporting to health supervision and inspection. |
