The Study Of The Combination Of The Fire-needle And Massage Therapy On The Shoulder-Hand Syndrome After Stroke

Objective:This study adopts randomized controlled trials to observe and evaluate the clinical effect of the therapy of fire-needle combining with massage on shoulder-hand syndrome(SHS)after stroke,and tries to analyze the mechanism and possible problems.It could provide references for systematic and standardized clinical treatment of SHS.Methods:The cases of this study are collected from the rehabilitation department of the Guangdong hospital of traditional Chinese and western medicine.The observation time is from December 2016 to December 2017.There are 60 cases dividing into the treatment group and the control group with random number table.Each group includes 30 cases.The treatment group receives massage once every day and fire-needle once every 3 days.Meanwhile the massage will be performed before the fire-needle.The treatment takes 1 day off every week.The control group receives normal acupuncture once every day.The treatment takes 1 day off every week.Both groups receive basic drug therapy and routine rehabilitation training.Every cases receive 1 course of treatment as 3 weeks.And the efficacy will be evaluated after the course.The clinical observation indexes include the visual analog pain scale(VAS),the simplified Fugl-Meyer movement function rating scale(upper)(FMA)and the modified Barthel index rating scale(MBI).Results:There are no significant differences between the general materials of two groups by statistical analysis including the gender,the age,the SHS stage,the TCM syndrome types,and the VAS,the FMA,the MBI scores before treatment(P>0.05).It indicates that the baseline data of two groups are consistent.These two groups are comparable.After the treatment,the VAS scores of two groups both decline,while the FMA total scores,subitem scores and the MBI scores rising.There are no statistically significant differences(P<0.05).It indicates that two treatments are both effective.The VAS scores of the treatment group decrease more than the control group,while the FMA total score rising more.There are no statistically significant differences(P<O.05).The cooperative movement with flexor,the cooperative movement with extensor and hyperreflexia improve most significantly.It indicates that the treatment group is more superior to the control group in pain control and improving the function of the upper limb.The MBI scores of the treatment group increase more than the control group.But there are statistically significant differences(P>0.05).It indicates that the efficacy of two groups in improving daily activity are similar.The total effective rate of treatment group are higher than the control group.There are no statistically significant differences(P<0.05).Conclusion:Fire-needle combining with massage treating SHS after stroke has good curative effect.There are more advantages comparing with common acupuncture in pain control and the function of the upper limb.This treatment is positive to improve the rehabilitation and survival of SHS after stroke,which is worthy on clinical research and promotion.
