Objective: to compare the clinical efficacy of divergent extracorporeal shock wave and local blocking in the treatment of calcified supraspinatus tendinitis at acute stage. To study the clinical effect of divergent extracorporeal shock wave in the treatment of acute stage of the disease,to provide a feasible scheme for the treatment of the disease,to further improve the treatment of the disease, and to improve the clinical effect of the treatment. Methods: Twenty-five patients with calcified supraspinatus tendinitis from October 2016 to October 2017 were analyzed. In group A,13 cases were treated with shock wave and 12 cases in group B were treated with local blockage. Before treatment,VAS score and UCLA score were used.Group A patients with shoulder tendon injury, shoulder periarthritis, other rotator cuff calcified tendinitis, other shoulder diseases and contraindications were treated with divergent extracorporeal shock wave, fixed parameters were set,and fixed parameters were taken.Sit at the end of the body, relax the shoulder joint, take the body surface mark and the pain point localization method, treat the center of the focus of calcified supraspinatus tendinitis with the probe to do the circle motion repeatedly, treat every other week,A total of 3 times of treatment.Group B was treated with local local blocking of ropivacaine mesylate injection and compound betamethasone injection. At most two needles. The patients in both groups were given functional exercise according to plan after treatment. The VAS score and UCLA shoulder function evaluation were recorded before, after the last treatment,1 week after the last treatment,2 weeks after the last treatment,4 weeks after the last treatment, and 4 weeks after the last treatment. The maximum transverse meridian of calcified foci in different time periods were measured and the data recorded in each stage of the two groups were compared and analyzed.Results:1? there was no significant difference in sex, course of disease, age and location of calcification between the two groups(P > 0.05).2.According to the VAS score, the P value of the two groups was more than0.05 at every time after treatment, but before the treatment of the same group,Four weeks after the last treatment,P values were all less than 0.05.The results showed that both shock wave therapy and blocking therapy could relieve or even eliminate pain, but there was no significant difference between the two treatments in alleviating shoulder joint pain.3.Compared with UCLA score, there was no significant difference between the two groups before treatment(P > 0.05),which indicated that the UCLA functional scores of the two groups were similar before treatment, and could be studied in the next step: after the last treatment,1 week after the last treatment, there was no significant difference between the two groups. The contrast P value was more than 0.05 in each period of 2 weeks after the last treatment, but P < 0.05 in the UCLA function contrast at 4 weeks after the last treatment, and P < 0.05 before and 4 weeks after the last treatment in the same group, which indicated that the two treatment methods could achieve good clinical effect. Both of them could improve the shoulder function, but shock wave therapy was more effective than that of blocking therapy.Treatment can better promote the recovery of shoulder function.(4)the disappearance rate of calcified foci was compared between the two groups(P > 0.05)at 1 week and 2 weeks after the last treatment, but P < 0.05 at 4 weeks after the last treatment, and before treatment in the same group.4weeks after the last treatment,P < 0.05,which indicated that both treatments could promote the disappearance of shoulder calcification, but the shock wave treatment group could promote the absorption of shoulder calcification better than the closed group. Conclusion: both extracorporeal shock wave and local blocking therapy are effective methods for the treatment of calcified supraspinatus tendinitis in acute stage. But the former treatment of calcified supraspinatus tendinitis in the acute phase can better promote the absorption of calcification, can better restore the shoulder motion, improve the function of the affected shoulder.
