Objective:To analyze the efficacy of simple nerve block and divergent shock wave combined with nerve block in the treatment of low back myofascial pain syndrome.Methods:88 patients with lumbago and fascia pain syndrome(L3 site pain)received in the pain department of our hospital in November-2017 August 2016 were selected to be divided into control group(simple nerve block group,n=44)and observation group(simple nerve block + divergent shock wave nerve block group,n=43).All patients received basic therapy(hyperthermia and lumbar dorsal muscle function exercise),and a mixed analgesic solution made of 2%lidocaine,methylprednisolone sodium(methylprednisolone)40mg,Mecobalamin(Mi Kebao)0.5mg,and physiological saline was injected into the tip of the third lumbar transverse process with local nerve block injection,2 times a week,with a total of 4 Weeks,the observation group received 1 rounds of extracorporeal shock wave treatment for 4 weeks.The visual analogue scale(VAS)score and infrared thermography were used to evaluate the pain of the lesions and the adverse events were recorded before each treatment.After the treatment,the patients were followed up for 4 weeks,and the degree of rehabilitation was evaluated by VAS score.Results:All patients were followed up after a course of treatment.Compared with the control group,the VAS score of the observation group decreased significantly at first,second and third weeks(First weeks:the observation group was 5.04 + 1.32 points,and the control group was 5.52 + 0.97 points;second weeks:the observation group was 3.25 + 0.96,the control group was 3.86 + 1.04;third weeks:the observation group 2.11 + 0.89 scores,the control group of 3.32 + 0.97 points;P<0.05).Group 3.32 + 1.02 points;P<0.05);The temperature region temperature of the observation group decreased significantly at second,third and 4 weeks in the observation group(second weeks:the observation group was-1.30 + 0.46,the control group was-1.410.62;third weeks:the observation group was-0.67 + 0.47,the control group was-0.79 + 0.55;the observation group was-0.41 + 0.49,the control group-0.51 + 0.50;P<0.05).After 4 weeks of treatment,87 patients were followed up.The VAS score of the observation group was still better than that of the control group(Observation group:0.31 + 0.47;the control group was 0.56 + 0.62).The two groups compared the VAS score and the infrared thermogram with the control group(P<0.05).Conclusion:The effect of extracorporeal divergent shock wave(ESWT)combined with local nerve block is more accurate than that of simple local nerve block,which is a new method for the treatment of lumbar and dorsal mryofascial pain syndrome.
