| Objective In this study,the clinical effect of local sealing combined with external application named Wuwei Guju Badu Powder and arthroscopic treatment of Calcified Supraspinatus Tendinitis(CST)was compared and analyzed,and the mechanism of external application of Traditional Chinese Medicine was explored.At the same time,it also provides a more reliable method for the treatment of this kind of disease,and provides some valuable reference for the clinical treatment.Methods The study chose the cases from January 2017 to December 2018,patients with Clcified Supraspinatus Tendinitis were treated in the First Affiliated Hospital Of Anhui University of Traditional Chinese Medicine.A total of 30 cases were selected,including 13 males and 17 females;12 cases of left shoulder and 18 cases of right shoulder;the age was 35-74 years;the mean age was(53.42 ±6.28)years old;the lasting course of disease was 1.5-6 years with an average of(3.47 ±1.02)years.They were randomly divided into two groups.15 cases in each group: the treatment group was treated with Wuwei Guju Badu Powder after local sealing,while the control group was routinely treated with shoulder arthroscopy clearance under the mirror.Gender,age,shoulder type,lasting course of disease and VAS pain score,shoulder function score were statistically analyzed to eliminate the difference.The treatment group was treated with anti-inflammatory and analgesic drugs after local sealing.After local sealing,the patients in the treatment group were treated with Wuwei Guju Badu Powder after local sealing in our hospital.Honey was applied and added to the affected area for as long as 12 hours,and the treatment period was 7 days.At the same time with functional exercise,mainly to active functional exercise,such as climbing wall with fingers,behind stretching,and so on.The control group was treated with arthroscopic cleaning,and the anti-inflammatory and analgesic drugs were given orally after the conventional operation.Two groups of patients were observed and recorded before treatment,the first treatment,the second course of treatment including shoulder pain score 2 months after treatment,shoulder Constant-Murley(C-M)score,shoulder motion score,ADL score.SPSS21.0 statistical analysis software is used to analyze the data.The metrological data were expressed as "mean standard deviation".Before and after treatment,the matched T test was used to compare the treatment scores between the two groups and the independent sample T test was used to compare the therapeutic scores between the two groups.The counting data were checked by chi-square test.The difference was statistically significant(P < 0.05).Compared with the two groups of experimental data,so that the clinical efficacy of the two treatment methods assessment.Results The general baseline data of the two groups,such as age,gender,shoulder type,duration of disease and so on,were compared by statistical analysis(P > 0.05).Shoulder pain score before and after treatment The VAS scores of the two groups before treatment were statistically analyzed,the results showed that P > 0.05,indicated that the result was comparable.The first course of treatment,the second course of treatment,the score of VAS at 2 months after treatment and the score of VAS of the two groups before and after the treatment were statistically analyzed and compared.The results showed that there was a significant difference between the two groups(P < 0.05).The result also showed that:(1)The two treatment methods had therapeutic effect on CST.(2)Compared with the control group,the effect of pain relief in the treatment group was better than that in the control group(P < 0.05).No matter in short-term or long-term pain relief,the treatment group has more prominent clinical efficacy.The scores of Constant Murley(C-M)of shoulder joint before and after the treatment The scores of two groups of patients(C-M)were statistically analyzed before and after treatment.The results showed that the scores were comparable(P > 0.05).The first course of treatment,the second course of treatment,the VAS score 2 months after treatment and the VAS score before and after treatment were statistically analyzed and compared between the two groups.The results showed that there was a significant difference between the two treatments(P<0.05).It announced that:(1)Both of the two treatment methods were effective in treating the shoulder motion limitation symptom of CST,and both of them could have definite clinical effect on the shoulder motion limitation symptom to some extent.(2)Treatment.Compared with the control group,the results showed that the treatment group was better than the control group in improving the motion of shoulder joint and avoiding the stiffness of shoulder joint.Scores of ADL before and after treatment Before and after treatment,the scores of ADL in the two groups were statistically analyzed,and the results showed that the scores were comparable(P > 0.05).The first course of treatment,the second course of treatment,the score of self-care ability in 2 months after treatment and the statistical analysis and comparison between the two groups before and after treatment.The results showed that there was also a significant difference between the two groups(P < 0.05).It pointed out that:(1)The self-care ability of the two groups was improved after treatment.(2)Compared with the control group,the results showed that the treatment group had better self-care ability than the control group(P<0.05),which indicated that the treatment group was better than the control group in improving the self-care ability of life.It has more prominent clinical effect.Comparison of total effective rate before and after treatment 6 patients were cured,3 were effective,4 were partial effective,2 were ineffective,and the total effective rate was 86.7 percent in the treatment group,while in the control group,4 patients were cured,2 were significantly effective,and 4 were effective,5 were ineffective.The total effective rate was 66.7%.The results showed that the total effective rate of the treatment group was significantly higher than that of the control group(P<0.05).Conclusion The combination of local sealing and external application of Wuwei Guju Badu Power and arthroscopic debridement under shoulder arthroscopy both had a remarkable clinical effect in the treatment of CST,but the former plays a even more important role in the treatment of the disease.After local sealing,the external application of Wuwei Guju Badu Power can obtain a good experience of instant relief of the disease immediately,effectively alleviate the symptoms of dyskinesia after operation,and obtain a faster recovery.Based on this,local sealing combined with external application of Traditional Chinese Medicine as a breakthrough point,for clinical diagnosis and treatment of Calcified Supraspinatus Tendinitis to provide a reference method.At the same time,it can also provide some useful reference and guidance for the future clinical work. |
