ObjectiveThrough the analysis of clinical data of patients with supraspinatus tendinitis and the principle of randomized controlled trial, the degree of shoulder pain, daily living ability and shoulder joint activity and functional score of patients before and after treatment were statistically analyzed, and the clinical efficacy of needle-knife therapy for supraspinatus tendinitis was observed and evaluated objectively.MethodsTo observe 60 patients with supraspinatus tendinitis who met the inclusion criteria in the Rocket Army Special Medical Center of the Chinese People's Liberation Army.According to the randomized clinical trial method, they were divided into needle-knife group and acupuncture group,30 cases in each group.Acupotomy group selected 5 pain points on the shoulder and treated them with acupotomy on the 1st and 5th days for 2 times. In the acupuncture group, the affected shoulder shaft, shoulder thigh, shoulder thigh, arm thigh, shoulder virginity and acupuncture points were selected for 20 minutes, starting from the first day of treatment, once a day for 5 consecutive days, with a total of 5 times of treatment. The patients in the two groups had a rest for 2 days after the course of treatment, and the clinical effect was observed and evaluated on the 8th day. After collecting the VAS scores of all patients' shoulder pain visual analogue scale before and after treatment, recording the ADL scores of patients' daily living ability before and after treatment, and measuring and evaluating the scores of patients' shoulder joint activity and functional ROM,the data are collated for statistics, and the clinical significance of the representative is analyzed according to the calculated statistical results, and finally the curative effect evaluation results and conclusions of the clinical study are obtained.Results(1)Comparison of VAS scores of shoulder pain in the two groups: The comparison of changes in VAS scores of shoulder pain before and after treatment in the acupotomy group and the acupuncture group were statistically significant(P=0.000<0.01),that is,acupotome treatment and needle There was a significant difference in the improvement of shoulder pain in patients with thorn treatment. There was a statistically significant difference in the VAS score between the two groups after treatment(P=0.000<0.01).The needle-knife group was superior to the acupuncture group; There was a statistically significant difference in the difference of VAS scores(P=0.000<0.01),and the needle-knife group was superior to the acupuncture group.(2)Comparison of ADL scores of daily living ability between the two groups: The comparison of ADL scores of daily living ability of patients in the acupotomy group and the acupuncture group was statistically significant(P=0.000<0.01),namely acupotome treatment and needle There was a significant difference in the improvement of daily living ability of patients with thorn treatment;there was a statistically significant difference in the ADL score between the two groups after treatment(P=0.000<0.01).The acupuncture group was superior to the acupuncture group; There was a statistically significant difference in the difference in ADL scores(P=0.000<0.01),and the needle-knife group was superior to the acupuncture group.(3)Comparison of shoulder mobility and functional ROM scores between the two groups: the comparison of shoulder mobility and functional ROM scores before and after treatment in the acupotomy group and the acupuncture group were statistically significant(P=0.000<0.01).That is,acupotome treatment and acupuncture treatment have significant differences in the improvement of shoulder mobility and function;there is a statistically significant difference in ROM score between the two groups after treatment(P=0.000<0.01).In the acupuncture group, there was a statistically significant difference(P=0.000<0.01)between the two groups before and after treatment, and the needle-knife group was superior to the acupuncture group.(4)Comparisons of curative effect between the two groups: Acupotomy group treated 30 cases,9 cases were markedly effective,21 cases were effective, the marked efficiency was 30%,the total effective rate was 100%;Acupuncture group treated 30 cases,1 case was markedly effective,19 cases were effective,10 cases were ineffective, the marked efficiency was 3.3%,the total effective rate was 66.7%.The marked efficiency and total effective rate of the needle-knife group were significantly higher than those of the acupuncture group. The marked efficiency of the two groups was significantly different(P=0.012<0.05),and the needle-knife group was better than that of the acupuncture group. The total effective rate of the two groups was significantly different(P=0.001<0.01),and the needle-knife group was significantly better than that of the acupuncture group.Conclusions(1)Acupotomy and acupuncture therapy can treat supraspinatus tendinitis, alleviate pain, improve the ability of daily activities, increase the range of motion of joints, and the treatment is safe and reliable.(2)Acupotomy is more effective than acupuncture in the treatment of supraspinatus tendinitis, and there are significant differences in the marked and total effective rates.(3)It is more effective in improving shoulder joint mobility, shorter course of treatment, higher efficiency, simple operation, safety and reliability. It is worth popularizing in the army and clinic.(4)The shoulder joint MRI has important guiding significance for the clinical diagnosis and treatment of supraspinatus tendinitis.
