| Objective:To observe the efficacy of different energy extracorporeal shock wave on shoulder disorders and explore the optimal energy of extracorporeal shock wave on the treatment of various shoulder disorders.Methods:From September 2016 to December 2019,376 patients with rotator cuff in-jury(139 cases),long bicipital tenosynovitis(127 cases)or calcified supraspinatus tendini-tis(110 cases)were treated in Department of sports medicine and joint surgery of Jilin Prov-ince People’s Hospital.All the patients of each disease were randomly divided into 4 groups according to the different energy levels of the Dornier Aries intelligent zoom shockwave therapy instrument(level 5:0.062m J/mm~2;level 6:0.084m J/mm~2;level 7:0.096m J/mm~2;level 8:0.117m J/mm~2;).Each groups was randomly divided into 2 groups according to the number of treatment pulses,and each disease group was divided into 8 groups.Each group was treated with extracorporeal shock wave therapy(ESWT)with the corresponding energy levels and pulses.The visual analogue scale(VAS)and Constant-Murley score(CMS)were used to determine the efficacy before treatment,1 week,2 weeks and 4 weeks after treat-ment.Results:There were no serious adverse reactions and complications occurred in all groups during treatment.In all groups of patients with rotator cuff injury,the VAS score and CMS significantly improved(P<0.05).and the CMS of group 3(0.096m J/mm~2;2000 pulses)showed greater improvement over other groups at 2 weeks and 4 weeks after treatment(P<0.05).In all groups of patients with long bicipital tenosynovitis,the VAS score and CMS significantly improved(P<0.05).and the group 6(0.084m J/mm~2;2000 pulses)showed great-er improvement over other groups at 2 weeks and 4 weeks after treatment(P<0.05).In all groups of patients with calcified supraspinatus tendinitis,the VAS score and CMS signifi-cantly improved(P<0.05).and the VAS score of group 8(0.117m J/mm~2;3000 pulses)showed greater improvement over group 4 and group 6 at 2 weeks after treatment,the VAS score of group 8(0.117m J/mm~2;3000 pulses)showed greater improvement over group 2 and group 6 at 4 weeks after treatment,the CMS of group 8(0.117m J/mm~2;3000 pulses)showed greater improvement over other groups at 2 weeks and 4 weeks after treatment(P<0.05).Conclusion:The extracorporeal shock wave therapy is effective in the treatment of shoulder disorders,which can effectively relieve the pain,recover the function of the shoul-der joint and improve the life quality of patients.The ESWT is safe and reliable.It has a better effect when the treatment in a energy level in 0.096m J/mm~2 and 2000 pulses on rota-tor cuff injury,while in 0.084m J/mm~2 and 2000 pulses on long bicipital tenosynovitis,and in 0.117m J/mm~2 and 3000 pulses on calcified supraspinatus tendinitis. |
