Abstract/Summary:
Objectives1.To translate the Hospital Ethical Climate Survey(HECS)into Chinese from English, and evaluate the reliability and validity of chinese versionof HECS scale.2.To explore the impact of the status of nurses’ perception of HECS on Organizational Citizenship Behavior(OCB),and discuss the mechanisms of each other.3.To attempt to find out how to promote nurses’ OCB by improving the nurses’ perception of HECS MethodsThe study is divided into two parts, the first part is the Chinese version of the HECS,the second part is to research the effects of nurses’ perception of HECS on OCB.The first part: first of all, achieving the authorization of the author of the HECS scale, then using the translation,back-translation,cultural adjustment and preliminary investigation to introduce the HECS scale into China.Choosing 429 nurses of top three hospitals, using Cronbach ’s alpha to measure the internal consistency, using test-retest reliability to measure the stability;using content validity index(CVI)to measure the content validity after the author’ permission, using factor analysis to measure the construct validity of the scale. The second part: a cross-sectional study was adopted.660 nurses were chosed by cluster sampling from top three hospitals chosed by random sampling.General information questionnaire,Hospital Ethical Climate Survey, organizational citizenship behavior scale were used to investigate the status of nurses’ perception of hospital ethical climate and OCB,to test the relationship between them.Using SPSS21.0,Epidata software to analyze the data. The statistical methods included descriptive analysis,Pearson correlation, and multiple linear regression.ResultsThe first part: the Chinese version HECS suitable to China’s cultural background was formed included 25 items and five dimensions. The correlation coefficient between dimension was 0.314~0.638(P<0.01),and between various dimensions and total score was0.650~0.845(P<0.01);The Cronbach ’s alpha was 0.915,and each dimension of Cronbach’ s alpha coefficient ranged from0.603 to 0.843,the test-retest reliability was 0.786;the I-CVI of Chinese version of the HECS were ranged from 0.83 to 1.00,and the S-CVI was 0.89;Exploratory factor analysis(EFA)could get five fators, namely, the relationship between hospitals, patients, doctors, nurses, and managers. The cumulative variance contribution rate was 59.45%.The result of Confirmatory factor analysis(CFA)of the five factors was that the factor model fitting is good, and all items load was above 0.4,fitting index χ2/df=2.100,RMSEA=0.063,GFI=0.870,CFI=0.894,IFI=0.896,RMR=0.028.The second part: the average scores of HECS was(4.30±0.44),OCB was(4.42±0.42).The correlation coefficient between nurses’ perception of HECS and OCB was 0.359(P<0.01).Nurses’ perception for the relationship between managers, patients and nurses could explain 23.1% of altruistic toward colleagues;Nurses’ perception for the relationship between nurses, hospital, doctor could explain 21.2% of organizational identification;Nurses’ perception for the relationship between hospital, nurses and doctors can explain 12.3% of conscientiousness;Nurses’ perception for the relationship between managers, doctors can explain 7.6% of interpersonal harmony;Nurses’ perception for the relationship between managers, nurses and doctors can explain 6.6% of protection company resources.Conclusion1.The Chinese version of HECS had good reliability and validity, and can be used to measure nurses’ perception of HECS.2.Manager should focus on the change of nurses’ perception of HECS,and take corresponding nursing measures to build good relationship between nurses and nurses, patients, hospital, doctors and managers to promote the nurse to make more beneficial behavior to the organization.
