Diabetes is one of six common geriatric chronic disease in our country,it is a high prevalence of permanentdisease,need long-term treatment and nursing.Because of our country medical health care capacity lack of funding,uneven development and lack of the management information system,and people in china prefer to live in their home,all kinds of continuity care model arises at the historic moment.The article is commence to the existing problems from the early prevention and nursing of diabetes,studied the different families living environment of the diabetic population through literature reading,field observation and in-depth interviews,rethink the elderly diabetes care in service design method.On the basis of the principles of service design,classified and analysised the senile diabetes care in China.Hope to replace the professional nurse's function with the current information about the Internet,to improve the life quality of old people after discharge.
