Chronic kidney disease is characterized by high prevalence,low awareness,poor prognosis and high medical costs.It is another serious disease that endangers human health after cardiovascular and cerebrovascular diseases,diabetes and malignancies.At the same time,due to the lack of a unified and complete information technology standard in the country,data sharing between hospitals and the mobile medical field can't be conveniently and quickly shared.Chronic kidney disease patients with out-of-hospital health management process lack of clinical data during the hospital data support,and the lack of long-term effective treatment of hospital data during the monitoring of the vital signs,are not conducive to the control of patients with CKD progress,it is difficult to effectively improve the quality of life of patients.In order to solve the above problems,make better use of hospital clinical data and extra-hospital physical data to help patients with kidney disease to manage their health,this paper presents a FHIR-based data sharing scheme for hospital and outpatient and an underlying data standardized storage scenario defined as a FHIR"resource",and on this basis,design and development of a chronic kidney disease management system.The system opens the FHIR API.The hospital clinical data is converted to the system database after converting the data through the API gateway according to the FHIR standard.Meanwhile,the hospital can also obtain the long-term sign data information of patients outside the hospital by calling the system API.The system is divided into two parts:the doctor end and the patient end,which provide patients with functions such as physical sign input,online real-time counseling,health plan guidance,message reminder and forums to help improve the level of chronic kidney disease control and ensure the quality of life of patients.
