| ObjectiveBased on the advanced methods to assess the status of immune function and the degree of infection at home and abroad,the optimal score method and related indexes were selected by examining a large number of literatures to create a scientific,comprehensive and reasonable computer digital module which was evaluated the immune function and degree of infection in patients with CKD.And the two test systems were applied to clinical clinicians and provide the objective basis for assessing patient status accurately,adjusting drugs,and assessing the treatment and prognosis.Method1.The create of digital module to judge the immune function state and degree of infection of patients with chronic kidney.(1)Retrieval database both at home and abroad,consulting the related literatures,books,minutes of meeting,collecting,sorting and screening the commonly used criteria,the practical index,testing method and the normal value to evaluate the immune function state and degree of infection.(2)Edit evaluation system with 10 million pixel digital camera shooting landscape image with the required medical images,Photoshop software processing and sorting pictures.(3)Debug computer module: debug this module at many times,to ensure that the module can run normally in different types of computer;then package the system with encryption and install in a dedicated computer after debugging qualified(teacher).2.Clinical application research(1)The edited computer digital module was used in clinical to verify its accuracy and comprehensiveness by comparing with general forecast model and the actual result.Adopt randomization,retrospective collection patients with CKD admitted in the second people's hospital of guangdong province,entry the patients indexes into measurement and control system software by two operators at the same time,run and calculation,and records the conclusion of each prediction system.Use the Excel form to organize the data.(2)Use SPSS 22.0 statistical software for statistical analysis of the collected data,various prediction systems were carried on evaluating diagnostic test and ROC curve study.Results1.Research and plait TIFS-CS&DJ systems,including two subsystems,respectively is the IFRB-S&DJ and T/B/NK DJ&AForIF-S&DJ.2.Research and plait TCIS-CDigCS&DJ,including eight subsystems,respectively is the IPS&DJ,CPIS&DJ,CAPSI-CS&DJ,The Sepsis S&DJ,systemic inflammatory response syndrome scoring system,TTR event-S&DJ,CKDIR S&DJ,Wu 'sCDwIS S&DJ.In addition to IPS & DJ system,the remaining nine systems can diagnose infection,the CAPSI-CS&DJ?The Sepsis S&DJ?CKDIR-S&DJ?Wu'sCDwIS-S&DJ and TCIS-CDigCS&DJ can assess the severity of infection.3.The plait system interface is clean,beautiful,the determining value is scientific,reasonable and comprehensive,the results and reports have detailed annotated,it can be automatically saved,transmitted and printed.After repeated comprehensive new investigation confirmed that there are still no identical or similar domestic and foreign reports,the creation of the system to fill the gaps at home and abroad in this field.4.T/B/NKIF-S&DJ and TIFS-CS&DJ systems have a good assessment of the status of immune function assessment.T/B/NKIF-S&DJ has a certain effect on predicting the risk of infection.The accuracy of each system diagnosis of infection from high to low respectively TCIS-CDigCS&DJ > Wu'sCDwIS-S&DJ > CAPSI-CS&DJ > CKDIR-S&DJ > ASS&DJ > CPIS&DJ > SIRSS-S&DJ > TTR-S&DJ,TCIS-CDigCS&DJ and Wu'sCDwIS-S&DJ sensitivity were higher than CAPSI-CS &DJ,at the same time on the degree of specific and accurate with CAPSI-CS&DJ.The accuracy of Wu 'sCDwIS S&DJ and TCIS CDigCS&DJ is better than CAPSI-CS&DJ and CKDIR-S & DJ on judging the severity of infection in patients.In summary,the plait systems of TCIS-CDigCS&DJ?Wu'sCDwIS-S&DJ in patients with the diagnosis of infection,severity assessment and prognosis are more accurate,scientific,reasonable,and has important clinical application value. |
