Objective:With the increasingly fierce competition in the medical market today,the comprehensive strength of the hospital cannot be achieved without the vigorous development of the nursing team.As the senior manager of the nursing team,the competency of the nursing directors affects the quality of nursing in the hospital.How to scientifically identify,select,and train outstanding nursing directors is a problem facing with nursing directors currently that needs to be resolved.The study aims to develop a questionnaire to test the reliability and validity of the questionnaire to form a scientific,simple,and feasible tool for assessing the competence of the nursing directors that is applicable to China's national conditions;and to use this questionnaire to assess the competency of the nursing directors in China.Conduct an objective and comprehensive assessment to understand the current status of the competency of the nursing directors,analyze the relevant influencing factors,and provide scientific basis for the selection,evaluation,assessment,and cultivation of related talents.Methods:The first stage: combining with research on competency at home and abroad through collecting a large number of Chinese and foreign literatures,using the evaluation index system of the Nursing Directors in the previous period of the research group,and initially drafted the dimensions and entries of the competency assessment questionnaire for the nursing directors.Through two rounds of expert consultations,the contents of the questionnaire were revised and improved.to determine the dimensions of the questionnaire and the weights of the items by using AHP.and a formal questionnaire on the competency of the nursing directors was initially formed.In the second stage,a questionnaire was used to conduct pre-surveys of the 12 level A Tertiary Hospitals caring managers in 6 provinces.The reliability and validity of the questionnaires were tested to form a questionnaire on the competency of the nursing directors.In the third stage,a formal questionnaire was used to survey the nursing directors in 32 Tier-3 hospitals in 16 provinces across the country to understand their competency levels,analyze their influencing factors,and finally draw conclusions.Results:1.The Nursing Director Competency Evaluation Questionnaire consists of 6 dimensions and contains a total of 27 items.2.Expert consultation method: The positive coefficient of experts for the two rounds was 100% and 78% respectively,and the enthusiasm of the experts was high;the authoritative coefficient of the expert group was 0.841,and the research results were reliable;the mean importance scores of all indicators were all> 3.50,coefficient of variation Both <0.20,no entries were deleted,one entry was added based on expert opinion,and some entries were appropriately modified;the degree of coordination of expert opinions after two rounds of expert consultation was significantly higher than that in the first round,with the authority coefficients 0.4674(dimensions),0.5840(entry),and significant test results P <0.05 respectively,the consensus of expert opinion is high.3.Analytic Hierarchy Process results: The weights of the first-level indicators are in descending order of personal quality and accomplishment(0.2842),management capacity and skills(0.2255),goal pursuit and action(0.1790),interpersonal communication and influence(0.1421),and service consciousness and guide(0.1054),professional knowledge and skills(0.0638).The top five items with the highest weight coefficient for the second-level indicators are decision-making ability(0.2000),emphasis on quality and order(0.1010),team leadership(0.0899),communication and coordination(0.0730),and training of talent(0.0621).4.Questionnaire reliability: The reliability of internal consistency was measured.The total Cronbach's ? coefficient of the questionnaire was 0.995,and the Cronbach's ? coefficient of each dimension was between 0.954 and 0.988.The questionnaire reliability was good.5.Validity of the questionnaire: The questionnaire was prepared by the Delphi method to ensure that each item can reflect the content of the forecast,and the content of the questionnaire is valid.The correlation analysis method shows that the correlation coefficient between the dimensions and the overall of the questionnaire is 0.926-0.984,and the correlation coefficient between each dimension is 0.840-0.961,and all the correlation coefficients are statistically significant(p<0.01).The correlation between each dimension is relatively high.Through confirmatory factor analysis,the load of all items in the questionnaire is greater than 0.6,CFI=0.997,TLI=0.996,RMSEA=0.040,90% confidence interval is within the accurate fitting range,WRMR value Close to 1.0,although there is a small deviation,other fitting indices are ideal.The questionnaire has a good structural validity.6.In the current status survey,the overall score of the Nursing Directors in China was 120.07±24.246.7.Population sociology factor of the nursing directors: there is no difference in the level of competence between the gender,marriage,and personnel status groups(p>0.05).The competency of the Nursing Directors differed in the following factors: different ages,years of primary health care work,years of nursing management work,academic qualifications,professional titles,and the number of provincial/national training posts in the past year(p<0.05).8.Multiple stepwise regression analysis of the total scores of the competency of the Nursing Directors and the scores of each dimension.The “age”,“primary years of nursing work”,“nursing management working years,” and “professional and technical titles” are the main influencing factors on the competency ratings of the Nursing Directors.Conclusion:1.The evaluation report of the Competency of the Nursing Directors is of good reliability and validity.It can not only provide an effective assessment tool for assessing the competence level of the Nursing Directors,helping the Nursing Directors to improve themselves and improving the performance of nursing management.but also providing scientific basis for the medical recruitment,performance appraisal,salary management,and provide important reference for the training of nursing management personnel in the health management department.2.The Nursing Directors in China is in a better position,but has a lower score in service awareness and orientation.It is recommended that relevant content training be enhanced to enhance the ability.3.It is recommended to strengthen the ability training for nursing directors who are engaged in primary care work and nursing management with shorter working lives and lower academic qualifications to improve the competency level of nursing managers in China.
