Objective: By observing the clinical efficacy of Longdan Mixture in the treatment of damp heat skin-enhancing skin eczema and its impact on the level of peripheral blood CD4 +,CD8 + T cells,explore the mechanism of drug action.Methods: Sixty-seven patients who met the criteria for case selection were included in this study.Randomly divided into treatment group(34 cases)and control group(33 cases).The treatment group was orally given compound gentian mixture,2times a day,1 bottle at a time,and 1 hour after meals,while taking a compound gentian mixture into warm water,diluted with a 1:2 ratio,and then soaked in a towel and warm and wet.Apply or scrub,once a day,obvious skin lesions can be used compound gentian mixture infiltration wet gauze,2-3 times a day.The control group was treated with mometasone furoate twice daily and applied to the skin lesions in appropriate amounts.Exudate obvious skin lesions with saline infiltration gauze wet dressing,2-3times a day.2 weeks for a course of treatment,a total of 2 courses of treatment.After the course of treatment,statistical analysis of the two groups of patients before and after treatment lesion area and severity scores,prancing degree scores and changes in CD4 +,CD8 + T cell levels,a comprehensive evaluation of efficacy,and record adverse reactions.After the end of the treatment,the recurrence rate was counted.At the same time,20 healthy subjects were included as blank groups,and the levels of CD4~+ and CD8~+ T cells were observed and compared with the two groups of eczema patients.Results:1.Of the 67 patients enrolled in this study,61 were required to complete all courses of treatment and follow-up as required,31 in the treatment group,30 in the control group,and 6 in the fallout.The shedding rate was 8.96%.2.The scores of EASI scores and pruritus severity scores of Longdan Mixture after two courses of treatment of acute eczema were significantly different from that of mometasone furoate(p<0.05).3.The total effective rate of Longdan mixture in improving skin lesion area and severity is 83.87%,which is obviously better than 73.33% of mometasone furoate.The total effective rate of Longdan mixture in reducing itching is 87.10%.70.00% of mometasone furoate.4.Before treatment,t test was performed on the levels of CD4~+,CD8~+,and CD4~+/CD8~+ T cells in the two groups,P=0.94,and there was no significant difference between the two groups.The mean levels of CD4~+ T cells in the two groups of eczema patients before treatment were significantly lower than those in the healthy blank group.There was a significant difference between the two groups in the paired t test(P<0.05);the mean CD8~+ T cell water level was higher than that in the healthy blank group.Statistically significant(P<0.05).After the end of the treatment period,the mean levels of CD4~+ T cells in the two groups increased significantly,and the mean value of CD8~+T cells did not decrease significantly.The mean water level of CD4~+/CD8~+ T cells increased significantly and returned to the normal range,both before and after treatment.Significant difference(P <0.01);treatment group than the control group.Conclusion: Longdan Mixture has a significant therapeutic effect in treating patients with damp-heat type eczema,and is more effective than mometasone furoate in improving the area and severity of skin lesions and reducing itching.Comparison of peripheral blood T-cell subsets in healthy individuals and eczema patients revealed that there were imbalances in the balance of CD4~+ and CD8~+ T-cell subsets in eczema patients,which may be one of the causes of eczema.After treatment,the mean value of CD4~+ water in the treatment group was significantly increased,and the level of CD4~+/CD8~+ was significantly increased,indicating that Longji Mixture had the function of regulating immunity.
