Clinicai Research Of Seton Technology Inside Rectum To Treat Abscess Of The Retrorectal Space

ObjectiveThis topic is to study the treatment of abscess of the retrorectal space by using seton technology inside rectum. By evaluating seton technology inside rectum and low incision and high catheter drainage of cure rate, postoperative pain, operation time, healing time, postoperative anal shape and function problem, after abscess recurrence rate and incidence of anal fistula of kin, etc.Discuss the advantages of seton technology inside rectum to treat abscess of the retrorectal space, so as to can be popularized in clinical application in the future.MethodThe 36 patients of the study from the department of Coloretal Surgery in Guang'anmen Hospital.They are in accordance with the diagnostic criteria of abscess of the retrorectal space.According to the prospective randomized controlled study,they randomly divided into two groups, 18 cases respectively.The observation group was used seton technology inside rectum . The control group was used low incision and high catheter drainage. The study needs to observe and record the two groups of cure rate,postoperative pain, operation time, recovery time, anal shape functions of three months after operation and the abscess recurrence rates and fistula rate during the follow-up of 3 months.The results of the above recorded are analysed by using SPSS21.0. When P value is less than 0.05 , there are a statistical difference between the two groups, but if the P value is greater than 0.05,oppositely.Results(1)In the aspect of therapeutic effect,seton technology inside rectum compared with low incision and high catheter drainage,they have no statistical differences ( P ?1.000,P>0.05).Two groups of patients were all cured, shows that they have no difference in the treatment effect of abscess of the retrorectal space.(2)In the aspect of postoperative pain, two groups of patients were recorded the most serious pain rating of a day in the postoperative 1, 2, 3 week,counted the total number of each pain levels.In the postoperative 1 week,seton technology inside rectum compared with low incision and high catheter drainage,they have a statistical difference(P1=0.000.P<0.05),In the postoperative 2.3 week,seton technology inside rectum compared with low incision and high catheter drainage,they have no statistical difference(P2=0.337?P3=0.104,P>0,05). The conclusion shows that in the postoperative 1 week.seton technology inside rectum has obvious advantages in alleviate postoperative pain.In the postoperative 2.3 week, two groups have no obvious difference in alleviate postoperative pain.(3)In the aspect of the operation time,seton technology inside rectum compared with low incision and hiigh catheter drainage,they have a statistical difference( P0.000,P < 0,05). It suggests in the treatment of abscess of the retrorectal space, seton technology inside rectum compared with low incision and high catheter drainage,it have the advantage of obviously shorten the operation time.(4)In the aspect of the healing time,seton technology inside rectum compared with low incision and high catheter drainage,they have a statistical difference( P0.000,P < 0.05). It suggests in the treatment of abscess of the retrorectal space,seton technology inside rectum compared with low incision and high catheter drainage,it have the advantage of obviously shorten the healing time.Seton technology inside rectum can make the patients recover at an early date.(5)In the aspect of the anal function,the two groups of 3 months after operation,seton technology inside rectum compared with low incision and high catheter drainage,two groups of patients are all normal, shows that they have no obvious difference in the aspect of protecting the anal function.(6)In the aspect of the anus appearance, the treatment group with seton technology inside rectum,have no incision outside the anus, so the anus appearance completely normal;the control group with incision outside the anus,the postoperative 3 months of the anus has a linear scar.even trench scar. Local tissue fibrosis harden will be influence to the anus surgery again and be not easy to heal,It can cause anal discomfort and effect the beauty of the anus. But these problems cease to exist for using seton technology inside rectum.lt suggests in the treatment of abscess of the retrorectal space,seton technology inside rectum compared with low incision and high catheter drainage,it has a significant advantage in protecting anal morphology.(7) In the aspect of the recurrence rate and fistula rate, two groups have no abscess recurrence or anal fistula formation during postoperative follow-up of 3 months. It suggests in the treatment of abscess of the retrorectal space,seton technology inside rectum compared with low incision and high catheter drainage,there was no significant differences about the recurrence rate or anal fistula formation in the near future.ConclusionIn the treatment of abscess of the retrorectal space,seton technology inside rectum compared with low incision and high catheter drainage, both in the cure rate,recurrence rate,incidence of fistula and protect anal function ,are roughly same, show the feasibility and safety of seton technology inside rectum.In addition, in maintaining anal form complete, control of postoperative pain, shorten the surgery and recovery time, seton technology inside rectum is in the advantage. These are proved that its can be used in clinical, for relatively simple operation, little pain, and because of its accord with modern surgical minimally invasive concept, so worthy of popularization and application in clinic.
