ObjectiveIn order to know thermal moxibustion in prevention of urinary retention after radical hysterectomy (RH) and observe whether it can reduce the incidence of urinary retention after RH, and improve the patients’urine condition, which can provide a new idea to prevent the urinary retention after RH, besides, which can have thermal moxibustion provided a safe, reasonable, effective and economic theoretical foundation and practical basis for the postoperative urinary retention of rehabilitation in clinical.MethodsIn this study, the patients who met the inclusion criteria after RH because of cervical cancer or endometrial cancer in department of gynaecology, Guangdong Provincial Hospital of TCM from November 2013 to February 2015 were as the research object. By means of a prospective randomized controlled method,34 patients were randomly divided into two groups:control group (n=17), treatment group (n=17).The control group on the basis of routine nursing and treatment adopted the Tamsulosing Hydrochloride Capsules since 5 day after surgery till their bladder function return to normal. And the treatment group on the basis of the control group’s treatment was intervened by thermal moxibustion, one time a day,30 to 40 minutes at a time,3 days for one period of treatment. By observing after removaling catheter in a specified period of time, if there would be no difference in the incidence of urinary retention, as well as in patients’symptoms and signs, residual urine volume, the first time urination of time, average urine flow rate, the patients’ urine condition etc.The test data were evaluated by SPSS19.0.Results1. The incidence of postoperative urinary retention were 0%(0/17),23.53% (4/17)in treatment group and control group respectively. Compared with the control group, the treatment group was significantly lower, and significant divergence was shown between the two groups (P<0.05).2. The average urine flow rate of postoperative urination for the first time after Catheter removal were 9.44±3.38ml/s,4.75±3.55ml/s in treatment group and control group respectively. Significant difference was found in them(P<0.05).3. The urination scores of treatment group (3.29±3.04 scores) were significantly lower than the control group’s (8.35±4.09 scores), and there was significant difference between them (P<0.05).4. The residual urine volume after the first catheter removal were 34.04±51.15ml,150.64±176.11ml in treatment group and control group respectively (P<0.05). Evaluating their effect by the residual urine volume, in 17 patients of control group 5 cases were excellent,3 cases were effective,9 cases were ineffective, and its total effective rate was 47.06%. While in 17 patients of treatment group 14 cases were excellent,1 case was effective,2 cases was ineffective, and its the total effective rate was 88.24%. The curative effect of treatment group is superior to the control group’s, and there was significant difference between them (P<0.05).5. There were no significant difference in indwelling catheter days, time to the first postoperative voiding, the rate of re-indwelling catheter, the rate of urinary tract infection, the postoperative hospital stay Between the two groups (P>0.05).ConclusionsThermal moxibustion combined with Tamsulosing Capsules had a satisfactory therapeutic effect on urinary retention after RH,compared with Tamsulosin Capsules purely, the former had more advantages. Thermal moxibustion may reduce the incidence of urinary retention, increase the average urinary flow rate and promote the recovery of bladder function, thereby improving the patients’ urine condition. Thermal moxibustion can provide a standardized, individual, economic and effective treatment to prevent urinary retention after RH, which should be widely used for treating the disease.
