| Objective:To observe the effect of constant temperature thunder and fire moxibustion on preventing urinary retention after extensive hysterectomy for gynecological malignant tumor.Methods:This study was a randomized controlled trial.A total of 200 cases of gynecological malignant tumor patients undergoing extensive hysterectomy in a Grade A hospital in Gansu Province were selected as the research objects.The subjects were divided into the experimental group and the control group with 100 cases each according to the random number table method.The control group was given routine nursing.On the basis of conventional nursing,the experimental group selected Qihai,Zhongji and Guanyuan acupoints to implement constant temperature thunder and fire moxibustion.The treatment course of the two groups was the same: 2 times /d,14 days as a course of treatment,15~20min each time,a course of treatment.Clinical symptoms and signs of the patients were scored 3 days before,the day of catheter removal and the first day after catheter removal.Residual urine volume after catheter removal,the first urination time after catheter removal,bladder function score,days of indwelling catheter,efficacy evaluation,and incidence of urinary retention were recorded in all patients.In addition,the results of sleep and anxiety scores were assessed and collected before the first postoperative treatment and after 14 days of treatment,and the scores were evaluated by the Inpatient Nursing Satisfaction Scale after treatment.The adverse reactions of patients were observed and recorded.SPSS25.0 statistical software was used for statistical processing of the data.Results :1.Symptoms and signs scores: three days before the catheter removal after treatment,compared with the control group,there were no statistical differences in the symptoms and signs of patients in the test group in terms of urination intention,frequency of urination,urgency of urination,dysuria,lumbosacral pain,anal drop pain,abdominal distension and abdominal pain(P > 0.05).On the day of catheter removal and the first day after catheter removal,the scores of urinary desire,frequency of urine,urgency of urine,dysuria,lumbosacral pain,anal drop pain,abdominal distension and abdominal pain in experimental group were improved with statistical significance(P < 0.05).2.Total scores of symptoms and signs: there were statistically significant differences in total scores of symptoms and signs between the experimental group and the control group three days before,the day of and the first day after catheter removal after treatment(P <0.05).3.Residual urine volume after catheter removal: 14 days after treatment,compared with the control group,there was statistical significance in residual urine volume after catheter removal in the test group(P < 0.05).4.The first urination time after catheter removal: compared with the control group,the first urination time after catheter removal could be decreased in the experimental group,with statistical significance(P < 0.05).5.Bladder function scores: compared with the control group in improving the scores of patients’ bladder function,the experimental group was better than the control group,with statistical significance(P < 0.05).6.Days of indwelling catheter: 14 days after treatment,the number of days of indwelling catheter in the experimental group was statistically significant compared with the control group(P < 0.05).7.Efficacy evaluation: after 14 days of treatment,the significant efficiency of the two groups was 36.36%(36/99),18.18%(18/99),and the effective rates were 31.31%(31/99)and32.32%(32/99),respectively.Overall,the experimental group was better than the control group,with statistical significance(P < 0.05).8.The incidence of urinary retention: the incidence of 48% urinary retention in the control group was higher than that in the test group(P < 0.05),which was statistically significant(P < 0.05).9.Sleep score: compared with the control group,the improvement degree of sleep score of patients in the experimental group was higher than that in the control group(P < 0.05),and the sleep score decreased continuously compared with that before the first postoperative treatment.10.Anxiety symptom scores: 14 days after treatment,the experimental group compared with control group,the sleep time,waking up at night and wake up earlier than expected time,total sleep time,total sleep quality and anxiety the symptom such as emotion,Bai Tiansi sleep during the day and the total integral statistically significant,the effect of the treatment group was better than control group(P < 0.05).11.Nursing satisfaction: after 14 days of postoperative treatment,there was a difference in nursing satisfaction between the two groups.Both the experimental group and the control group could improve nursing satisfaction,and the constant temperature thunder and fire moxibustion group had more advantages(P < 0.05).12.Safety evaluation: during the whole treatment process,no patients in the constant temperature thunder and fire moxibustion group had local skin flushing and other adverse reactions,which was safe and effective.Conclusion:1.Constant temperature thunder fire moxibustion to prevent the treatment of patients with gynecological malignant tumor extensive postoperative urinary retention,can improve patients after pulling up the urine tube residual urine volume,pull out the first urination urine tube time,bladder function score,the number of days of placing a urinary catheter,so as to reduce the incidence of urinary retention,improve the effect of clinical treatment,comprehensive curative effect is better than that of control group,has clinical significance.2.In the process of preventing urinary retention after extensive surgery for gynecological malignant tumors,constant temperature thunder and fire moxibustion can effectively improve the scores of symptoms and signs of patients with urinary intention,frequency of urination,urgency of urination,dysuria,lumbosacral pain,anal drop pain,abdominal distension,abdominal pain,etc.,and it is better than the control group on the day and the first day after pulling out the urinary catheter.3.With the extension of treatment time,constant temperature thunder and fire moxibustion can effectively relieve postoperative urinary retention,at the same time,it is conducive to alleviate patients’ anxiety symptoms and promote postoperative rehabilitation.4.Thermostatic thunder and fire moxibustion can not only alleviate urinary retention after extensive surgery for gynecological malignant tumors,but also improve the nursing satisfaction of inpatients and establish a relationship of mutual trust between nurses and patients. |
