Objective:To get and analyse nursing scheduling situation in general wards of acomprehensive hospital in Shanghai which provide high quality nursing service, andfind the problems existing in the current situation of nursing human resource andnursing scheduling, in order to establish a classification system of general wardsnurses and collect the total working hours per shift in grade Ⅲ general hospital inShanghai;according to the standard ideal working hours in different levels of nurses,to construct nursing scheduling model in general hospitals which provide high qualitynursing service.Methods:1.On the basis of literature review and expert meetings, we designed thequestionnaire, all head nurses of39general wards were investigated in a generalhospital in Shanghai which provide high quality nursing service,we got to knownursing scheduling status and analyse the problems existing in the current situation ofnursing human resource and nursing scheduling.2.According to policy and expert meetings,to determine nursing division, entrycriteria, ability standard and responsibility.3. According to the result of measurement of working hours of each shift of allnursing staff,eventually we formed scheduling model of the high quality nursingservice in shanghai general hospitals.Results:1. Nursing staff allocation is unreasonable, mainly reflected in the allocationquantity deficiency, can’t meet the needs of patients for nursing care, nurses workingin overload, nursing scheduling satisfaction is low.2. The nursing staff are classified in two levels:auxiliary nurses,primarynurse,increarsing health care attendant.3. Time study method was used to collect the total working hours per shift andthe standard ideal working hours in different levels of nurses,in order to establishnursing scheduling in grade Ⅲ general hospital in Shanghai. 4. Establish a scheduling model of general hospital in Shanghai which providehigh quality nursing service.Surgical department nurse day shift for6-7patients, late shift is responsible for13-16patients, night shift is responsible for the15to16patients,can achieve theoptimal workload nurse standard;day shift for18patients, late shift is responsible for26-49patients, night shift is responsible for the62-113patients,can achieve optimalauxiliary nurse standard work.Non-surgical departments responsible nurses day7-8patients, the late shift isresponsible for the13to18patients,night shift is responsible for the13to18patients,can achieve the optimal workload nurse standard;day shift for15-20patients, late shiftis responsible for28-40patients, night shift is responsible for the58-70patients,canachieve optimal auxiliary nurse standard work.Conclusion:Nursing human resource is in shortage and scheduling is unreasonable,the lack ofscientific basis,can’t meet the needs of patients for nursing care in general wardswhich provide high quality nursing service.Through the scheduling model building,the purpose of this study is to meet the demand of rehabilitation treatment, improvethe quality of nursing service quality and patients satisfaction;at the optimum level ofnursing workers labor intensity,improve the level of clinical nursing ability;reduce thenurse high strength pressure load,improve high qualification nurses’ workingenthusiasm,improve nurses’ job satisfaction and a sense of achievement.
