Research On Evaluation Index System And Evaluation Model Of The Construction Of General Practice Department In Standardized Clinical Residency Training Bases(General Hospitals)

Background: To implement the reqirements of the Health and Medical Commission of China,General Hospitals(GH)responsible for Standardized Training of Resident(STR)from different areas of China are setting up General Practice Department(GPD).However,the questions of how to set up the department,or how to develop and evaluate it after it sets up have been raised.At present,construction and evaluation standards of other medical specialty departments have been applied to the newly setting up GPD,which are incompatible with its characteristics and must have formed constraints on the development.The issue of how to establish the evaluation mechanism of GPD in Standardized Clinical Residency Training Bases(General Hospitals)has become a new problem to be solved in the new era.Methods: In this study,the evaluation index system and evaluation model of GPD responsible for STR were formed by literature research,focus group discussion research,and delphi research.Firstly,the framework and content of index system is primarily designed through literature research and focus group discussion research.After that,indicators were screened,weights were estimated and models were determined by delphi research.All datum were built by Microsoft Word 2010 or Microsoft Excel 2010 and statistically analyzed by Microsoft Excel 2010 or SPSS 20.0.Results: Firstly,framework of evaluation index system was formed through the literature research.Then,the framework was improved through two rounds of focus group discussions.On this basis,we developed the evaluation index system(1st Edition)preliminarily.Next,45 senior general practice experts from 22 provinces,cities and autonomous regions were chosen to participate in three rounds of delphi research.Experts were asked to modify the evaluation index system and evaluation scheme,as well as to assign the weight coefficient.It finally built a three-leval index evaluation system and evaluation model around six first-leval indicators of "basic conditions","medical technology team","medical service capabilities and levels","medical service quality","teaching" and "scientific research".The positive coefficients of three rounds of experts were 84.44%,92.11%,and 91.43% relatively.The authoritative coefficient was 0.94.After the third round of consultation,the coefficient of expert coordination was 0.716,on behalf of the opinions of experts being consistent.Conclusions: 1.An evaluation index system for GPD in Standardized Clinical Residency Training Bases(General Hospitals)was established,which covered all aspects of the elements of general practice.The indicators were concise and to the point,as well as the weights were allocated reasonably.2.A comprehensive evaluation model for GPD in Standardized Clinical Residency Training Bases(General Hospitals)was constructed,including specific evaluation standards and scoring methods for each third-level indicator,which has good operability.
