An Investigation Into The Current Situation Of Sub-delirium Syndrome By Nurses In Intensive Care Unit Of General Hospital

Objective our study implement questionnaire about sub-delirium syndrome of general hospital ICU patients and then analysis of affecting factors on ICU nurses through literature review,Delphin Method,knowledge-attitude-belief-practice Model(KABP Model),to provide theoretical foundation for sub-delirium syndrome judgement improvement of ICU nurses.Based on nursery of delirium syndrome,it is significant clinical value for reducing occurrence of sub-delirium syndrome or deteriorating to delirium syndrome.Methods 1.Based on literature review,summary and analysis of previous research achievements,summarize expert advice and communicate with ICU nurses,undergoing by a two-round Delphi method based on literature review and questionnaire survey about KABP Model of sub-delirium syndrome,then consutation and demonstration of Zhejiang specialists,many indexes including expert familiarity coefficient,active coefficient,authority coefficient,variation coefficient,coordinate coefficient,test reliability and representativeness of expert consultation.After analysis,feedback advices collection,term manifest,representation and semanteme are modified,the content contains personal files and questionnaire survey of diagnosis,nursery attitude and nursery behavior about sub-delirium syndrome on general hospital ICU nurses(3 dimensionality&32 terms).2.After questionnaire survey of 170 general hospital ICU nurses,confirm reliability and representativeness of questionnaire via Cronbach's A index,retest reliability and content,structural validity analysis.3.questionnaire survey of KABP Model and influence factors analysis are implemented on ICU nurses in 9 Zhejiang general hospitals.Results 1.Accomplishment of questionnaire survey of KABP Model on ICU nurses in general hospitals including 3 dimensionality and 32 terms(15 terms of diagnosis about sub-delirium syndrome,7 terms of nursery attitude about sub-delirium syndrome,10 terms of comprehensive nursery behavior about sub-delirium syndrome).The questionnaire survey contains 5 levels scoring methods,total score is from 32 to 160.The nurses get higher scores,the levels of diagnosis,nursery attitude and comprehensive nursery behavior are higher.2.Our study indicates total Cronbach's A index of KABP Model on ICU nurses in general hospitals is 0.964,Cronbach's A index of diagnosis about sub-delirium syndrome ICU nurses in general hospitals is 0.976,Cronbach's A index of diagnosis attitude about sub-delirium syndrome is 0.934,Cronbach's A index of nursery behavior about sub-delirium is 0.904,test-retest reliability is 0.985,content validity score is 0.97-0.99,term and dimensionality,dimensionality and total score all relative high(P<0.001).Structure validity KMO=0,933,3 factors of all,cumulative contribution is 69.38%,consistent with survey structure,with better construction validity.3.Accomplishment of survey of KABP Model and influence factors analysis are implemented on 346 ICU nurses in 9 ZheJiang general hospitals,diagnosis,nursery attitude,nursery behavior and total score about sub-delirium syndrome are(40.75±14.10),(28.33±4.24),(36.07±6.99),(105.16±20.18),respectively.According to the multivariate linear regression analysis,age,work experience and professional title are major factors of KABP Model on ICU nurses.Conclusion l.The questionnaire survey of KABP Model on ICU nurses in general hospitals have good reliability and validity,is available for testing KABP Model about sub-delirium syndrome on ICU nurses.2.The level of KABP Model about sub-delirium syndrome on ICU nurses need to be improved.The age,work experience and professional title are major factors after influence factor analysis.As the age,work experience and professional title elevated,The level of KABP Model about sub-delirium syndrome on ICU nurses manifest an increasing trend.With influence factors,the nurse manager use specific interventions to improve judgement and diagnosis level of KABP Model about sub-delirium syndrome on ICU nurses.After that,ICU nurses could diagnosis quickly sub-delirium syndrome,reduce incidence of sub-delirium syndrome and raise patients' survival quality.
