Construction Of Transitional Care For Patients From Intensive Care Unit To General Ward

Objectives:1.The aim of this study is to assess the effectiveness of transitional care programs,to analysis the influence of different intervention forms,duration on intervention effect and to provide guidance for the construction of the transitional nursing intervention program.2.Based on the results of Meta analysis and previous literature review,a transitional nursing plan for patients transferred out of ICU was preliminarily constructed.Methods:1.SinoMed,CNKI,VIP,WanFang,PubMed,Embase,Cochrane Library and web of science databases were systematic searched,and the search period is from the establishment of the database to October 2019.Subject words and free words are combined,and a systematic literature search strategy is used to comprehensively search relevant randomized controlled studies.After the literature is screened,the original information related to the research is summarized.Cochrane reviewers’ Handbook was used to evaluate the quality of the included studies,and Revman5.3software was performed for Meta-analysis.2.Combined with the results of literature research and Meta-analysis,a preliminary draft of intervention content was formed.Using the Delphi method,13 nursing and clinical medical experts from third-grade class-A hospitals in Jiangxi Province were selected for two rounds of consultation.Based on the scientific and feasible principle,combined with the consultation of expert opinions,after discussion and modification by the research group,the final ICU transitional care content was formed.Results:1.Finally,13 studies were included in the Meta-analysis.The post-intervention effect evaluation showed that the transitional nursing intervention group was able to reduce the ICU readmission rate more than the control group.The results of thesubgroup analysis showed that there is no difference in the effect of different intervention forms on prognosis;the intervention lasting 7 to 8 days has a significant effect.2.In this study,the effective recovery rates for the two rounds of experts enquiry were 92.9% and 100% respectively,the expert ratios for suggestions were 76.9% and15.4% respectively,the authoritative coefficient was 0.81,and the familiarity was0.68.After two rounds of consultations by experts,7 items were adjusted,1 item was deleted,and 1 item was added.Finally,the intervention content of 32 items consisting of the intervention plan introduction and the intervention content was established.Conclusion:Based on the systematic review and Delphi method,the content of nursing intervention in the transitional period of patients transferred out of ICU is scientific and implementable,which can be used for reference in clinical nursing practice,and is conducive to better implementation of nursing transitions for patients.
