| Objective: To understand the needs of terminal cancer patients in general hospitals for physical environment in wards,and to build a localized physical environment solution for hospice wards,so as to provide theoretical reference for reforming hospice care ward in general hospital.Methods: Based on the literature study,a qualitative interview and expert consultation method was used to construct a physical environment protocol for hospice wards in general hospitals.The first stage used a literature research approach to extract domestic and foreign physical environment design elements.The second stage used descriptive research to conduct semi-structured,face-to-face individual interviews with terminal cancer patients in general hospital.The third stage used an expert-consultation approach,combining literature review and qualitative interviews to construct a physical environment protocol for localization hospice wards and to remould hospice wards in the oncology department of a general hospital.Results: 1.Domestic and international scholars have designed different intervention strategies for physical environment factors such as light,sound,color,nature,artwork,space(distribution,size and layout),decoration,etc..2.Terminally cancer patients have nine themes for their ward physical environment needs: home-like environment,sensory comfort,spatial comfort,exposure to nature,privacy and dignity,autonomy,medical and personal safety,social interaction,spiritual expression.3.After two rounds of expert consultation,the mean value of importance of the entries range from 3.85 to 5.00,the frequency of full score of importance of the entries range from 30.77% to 100%,the mean value of importance and the full score of importance met the entry criteria,indicating a high degree of concentration of opinions of consulting experts;The Kandall coefficient of concordace of item importance is 0.248,Chi-square test shows that the results are statistically significant(P < 0.001),and the coefficient of variation of each item is less than 25%(0 ~ 23.37%),experts tend to agree,and the final scheme contains 5 dimensions and 30 items.Conclusion: 1.Hospice design or renovation can take into account physical environment elements such as light,sound,color,nature,artwork,space(distribution,size and layout),and decoration.2.There are many demands for the physical environment of the ward for terminal cancer patients,and it’s necessary to build a ward environment for terminal cancer patients.3.The physical environment of hospice wards in general hospitals is guide by a theoretical foundation,and the contents are operable and practical,and can provide a theoretical reference basis for the reform or construct of hospice wards in general hospitals. |
