| As the most important institution of health risk management, medical care security is related to livelihood issues under the premise that health has been the indispensable component of civil rights. With the tireless efforts of the government, China has built up a basic medical care security system covering both urban and rural residents. Taking Basic Medical Insurance for Urban Workers, Basic Medical Insurance for Urban Residents and The New Rural Cooperative Medical System all into account, the whole urban-rural medical care security system has covered more than1295million people and its coverage has been higher than95percent according to the official statistics.Although China has got significant results in medical care security in the past decades, the urban-rural medical care security system exposes several unreasonable problems in the times that both industrialization and urbanization are developing rapidly. The fairness and accessibility of medical care security system has become worse. The operational efficiency of the medical care security system also is unsatisfactory, because the powers and responsibilities regularly overlap among several administrative management and service agencies. In order to solve all the problems above thoroughly, some reform and innovation appear to be very necessary for the current system. Obviously reforming the current uncoordinated system is the exact key point of this innovation.Focusing on the long development term, this article takes building integrated urban-rural medical care security management system as the goal of this research. Both theoretical discussion and case study are used in this article so as to get a more comprehensive analysis result. Under the clear understanding of development thread, the realistic dilemma in current system is smoothly sorted out by the Theory of Institutional Change. Some domestic and international revelation and experience also are very useful to build a more equitable system. At last, the policy framework and detailed path are successfully designed from the angle of Polycentric Governance. |
