Study On System Improvement Of The New Rural Cooperative Medical Scheme

The New Rural Cooperative Medical Scheme(CMS) is in contrast to the traditional Rural CMS. It is the medical mutual redress scheme for rural people, which is organized, guided and supported by the government, participated voluntarily by rural residents, funded by multiple parties including the individual, the collective and the government, focused on compensating catastrophic diseases. Seriously studying and solving the system improvement of the New Rural CMS is of great significance for the protection of the health of farmers , prevention poverty and re-poverty caused by illness, thereby contributing to social harmony and stability.The New Rural CMS should learn from the traditional Rural CMS experience and its lessons. Rural CMS under Collective economic, despite has many defects such as difficult to achieve sustainable development, financing in the single form, low level of protection, the poor inadequate way to solve the technical problems, but as a new thing, survived so many years under that prevailing socio-economic environment , has its own natural reasonable ingredients. Its way to finance (collectively economic as the main financial support), the reference standard (low threshold) and the principles (prevention), were all compatible with the prevailing economic and social conditions. Rural CMS mainly in personal burden on the whole is more harm than good, because there are many irrational aspects, such as lacking state government responsibility, lacking of effective management of the fund and abandoning the fine tradition "integration of Chinese and Western Medicine, prevention ", These resulted in the CMS difficultly recovered and developed. There are also certain advantages, particularly in stressing the importance of personal responsibility, has made a good bedding for the smooth implementation of the New Rural CMS.The reason why the New Rural CMS is "new", mainly in the following areas : participation in a new way, the new financing mechanism, new levels of co-ordination, the new focus of security, management structure and operation of new mechanisms. This is why the New Rural CMS has more vitality.Through demonstration of the New Rural CMS in Tai'an and Dongying, some achievements have been get and management system and operational mechanism which meet the realities has been initially formed. But there are still operating under many problems, including the difficulty in raising funds, the current funding level not enough to solve the problem of farmers medical care, the proportion of submit accounts and scheme still unreasonable, fund facing a large risk in the course of operation, publicity forms and efforts need to be further improved, control of the price of drugs and services has not been standardized, information systems and equipments been used inadequately, the number of the administrators is low ,the coverage is not comprehensive. These problems are seriously hampering the New Rural CMS developing persistently, necessary to adopt appropriate measures to be improved.Making full analysis to the historical lesson and existing problems, this article proposes to consummate the New Rural CMS such measures should be taken: further elaborate the leading role of the government; continue to make greater efforts to publicize ; further improve the financing mechanism and the reimbursement system, strengthen the management of the fund and risk prevention; enlarge the security coverage, and strictly control the medical costs of the excessive growth disorderly. Some corresponding measures should be taken: further reform of the medical care system, and further enhance rural health management integrated built, development of the rural economy to increase the income level of farmers, carry out active health education to peasants, improving the rural ecological environment.The division between urban and rural medical security system is not a permanent solution. Therefore the "three-step" line of the overall planning for urban and rural medical security system was put forward in this paper : the first step, from now to 2010, a medical insurance system which is full coverage but different between urban and rural will be achieved; the second step, from 2010 to about 2025, a medical insurance system which is plan as a whole in a province-wide ,with unified system but different standards; the third step, from 2025 to the middle of this century, a medical insurance system which is plan as a whole nationally ,with unified system but diverse forms.
