A Study On The Post-acute Care Demands And The Influence Factors Of Coronary Heart Disease Patients Within Two Weeks After Discharged From Hospital

Objective:By investigating the coronary heart diseases patients' health statuses when discharged from hospitals and the health service utilization in two weeks after discharges,to find out their health needs and demand,satisfaction degree of community health service utilization,then to make some objective suggestions for community health service promotion.Method:We selected the subjects through a cluster sampling procedure. first,we randomly selected 4 hospitals of Beijing,Shanghai and Nanning citys.,then we investigated the the coronary heart diseases outpatients matched the conditions of these hospitals.By case records' first page and self-made questionnaire,information was collected as follows:the discharged patients' generalized information,health statuses when discharging from hospitals, consultation and re-hospitalization in two weeks after discharge from hospitals, utilization and satisfaction degree of community health service.All the data analysis was done in SPSS13.0.Results:We investigated 316 the coronary heart diseases outpatients in all. After discharged from hospitals,the investigated patients' consultation rate was 47.15%and re-hospitalization rate was 28.2%in two weeks.The community consultation rate was 5.4%and the hospital consultation rate was 43.99%.In two weeks,44.6%of the respondents had taken family nursing,the average nursing time was 4.63 days,and 37.5%of all the respondents had purchased drugs without the prescriptions,the average frequency of purchase was 0.81 times.The employment status,education level,marital status,the way how to pay medical expenses and body functional status when discharged from hospitals were important influence factors of consultation rate in two weeks.And economic income was the important influence factor of re-hospitalization rate in two weeks.The satisfaction surveys showed that the majority of respondents regarded the medical technology level,service attitudes,medical costs,price of medicine and the environment of community health service were moderate.Conclusion:There is still much post-acute care demand in discharged patients with coronary heart disease.Combining the health care and nursing service of discharged patients with the development of community health care facilities, consummating the two-way referral system,specificating the community management of patients with chronic diseases and providing continuity care for discharged patients are important measures to utilize rational health resources reasonably and reduce the medical costs.
