Objectives: Mainly focuses on the evaluation and determinants of health care demand among the elder patients in discharge, on the analysis the insurance of post-acute care among the elder discharged patients transferred from hospitals to community health service institutes.Methods: Using literature review to summarize and analyze international experience on post-acute care patterns. Retrieving questionnaire replies to investigate four chosen tertiary hospitals in Beijing, Shanghai and Nanning to analyses 1) the demands of post-acute care of the elder discharged patients and 2) the cognition and evaluation of the elder patients in discharge on the community health services.Results: Based on the research, 1) most of the elder discharged patients are willing to get post-acute care, especially of those chronic disease patients; 2) most of them have good compliance to treatment in drug use; 3) most of the patients are not satisfied with the communities' medical technology and the staffs' initiative; 4) financing and payment methods should have played an positive role in guiding reasonable medical treatment; 5) doctors have extended their responsibilities in different aspects, as: health service pattern transformation, medical behavior re-building, and health education promotion.
