Approaches Of Transitional Care To Improve Self-management Level On Post Coronary Angioplasty Patients

[Objective] To study the influence of transitional care model on the self-management and illness perception among patients undergoing coronary intervention,in order to improve the prognosis of the disease and find the valuable experience of chronic disease management.[Methods] 77 patients who were implanted the coronary stent were randomly divided into experimental group(n=39)in which everyone would receive transitional care and the control group(n=38)in which everyone would receive conventional care to observe whether there is statistical difference between them.The observation was implemented three times when the patients left hospital,3 months later and 6 months later after they left hospital.[Results]The sociological and epidemiological of the patients had no statistical difference(P>0.05).The score of illness perception and self-management in the two groups shows no statistical difference.The self-management score of the control group(64.6�18.3)and the experimental group(64.2�15.4)were both very low,as well as the score of Illness perception(7.6�6.1vs7.6�4.7).Illness perception score of the experimental group were all higher than the control group(P <0.05)when the patients left hospital,3 months later and 6 months later after they left hospital.Self-management score of the experimental group were higher than the control group(P <0.05)3 months later and 6 months later after the patients left hospital.From the time tendency chart,the score of patients self-management which collected at the time of discharged and 3 months and 6 months after discharge shows escalating trend,and the trend are more obvious than the control group.[Conclusion]The illness perception and self-management level of post coronary angioplasty patients is generally low,we should pay great attention to the CHD patients.Transitional care can improve self-management level on patients after coronary artery stent implantation.
