Objective To describe occupational stress and job burnout situations of operating room nurses and the difference between such situations and that of other departments; to discuss the relation between occupational stress and job burnout of nurses and style A behavior, coping strategy, social support and general well-being ; to give theoretic basis for improving occupational stress and job burnout situations of operating room nurses, enhancing their working enthusiasm and initiative, and increasing living quality of nurses.Methods Using the method of sampling by layer, sampling all 174 operating room nurses of four hospitals with level III grade A in Jinan to form the observation team; dividing the nurses of the same four hospitals into layers according to various departments such as medicine department, surgical department, gynecologic department, pediatrics and emergency room, sampling 9 nurses that meet selection standards from each department randomly and total 180 nurses to form the samples of control group. Using Normal Situation Questionnaire, OSI-R Occupational Stress Inventory-Revised, MBI Maslach Burnout Inventory, Type A Behavior Pattern, Simplified Coping Style Questionnaire, Social Supporting Inventory and General well-Being Schedule to collect the data. Using SAS statistical software to finish data processing. Using frequency and percent to describe normal data; using t-test of two samples that use completely random design to compare the difference between two groups of nurses in general well-being, occupational stress, job burnout and style A behavior, coping strategy, social support; using t-test with independent samples to compare occupational stress and job burnout of two groups of nurses with normal reference; to compare job burnout situations of two groups of nurses in light, medium and heavy percents; using Pearson correlation to analyze the relation between general well-being, occupational stress and job burnout and the relation between general well-being, occupational stress, job burnout and style A behavior, coping strategy, social support; using multiple stepwise regression and using general well-being, occupational stress and job burnout as dependent variables, style A behavior, coping strategy, social support and normal data of nurses as independent variables to analyze influential situation and predication function of the independent variables on the dependent variables.Results1. The difference between general well-being, three sub-items of occupational stress, three sub-items of job burnout and style A behavior, coping strategy, social support of two groups of nurses. Score of operating room is higher than that of other departments in occupational role and objective support, P<0.05, the differences have statistical significance.2. The difference between 14 sub-items of occupational stress of two groups of nurses. Operating room is obviously higher than other departments in working environment, P<0.01; operating room is lower than other departments in sense of responsibility, social support and rational matter handling, P<0.01 or P<0.05; the differences have statistical significance.3. The difference of occupational stress between two groups of nurses and normal reference. Operating room nurses are obviously higher than normal reference in working environment, body stress coping and mental stress coping, P<0.01; other department nurses are obviously higher than normal reference in body stress coping, working environment, sense of responsibility, mental stress coping ana social support, P<0.01 or P<0.05; operating room nurses and other department nurses are obviously lower than normal reference in role stress coping, role illegibility, role discomfort and rational matter handling, human relation stress, role conflict, P<0.01. The differences have statistical significance.4. The difference of job burnout between two groups of nurses and normal reference. Job burnout level of operating room nurses and other department nurses is obviously higher than normal reference. Operating room nurses are higher than normal reference and other department nurses in emotion exhaustion; operating room nurses are higher than normal reference and lower than other department nurses in depersonalization; operating room nurses are lower than normal reference and higher than other department nurses in decreased personal accomplishment; P<0.01 or P<0.05. The differences are obvious and have statistical significance.5. The difference of job burnout of two groups. Compare job burnout situations of two groups of nurses in light, medium and heavy percents. Percents of middle and heavy emotion exhaustion of operating room nurses are obviously higher than other department nurses, P<0.01, the difference has statistical significance. Percents of heavy depersonalization of operating room nurses are obviously lower than other department nurses and percents of heavy decreased personal accomplishment are higher than other department nurses.6. Correlation analysis for general well-being, various sub-items of occupational stress and job burnout. General well-being have negative correlation with occupational role, personal strain, emotion exhaustion, depersonalization and have positive correlation with personal cope. decreased personal accomplishment, P<0.01 and has statistical significance.7. Correlation analysis for general well-being, sub-items of occupational stress, sub-items of job burnout and style A behavior, coping strategy , social support. General well-being has negative correlation with passive coping and has positive correlation with other factors, P<0.01 or P<0.05. Occupational role, personal strain, emotion exhaustion and depersonalization have positive correlation with passive coping, P <0.01 except occupational role; they have negative correlation with other items, among them, occupational role has negative correlation with active coping, style A behavior, subjective support and support utilization; personal strain has negative correlation with active coping, objective support, subjective support and support utilization; emotion exhaustion has negative correlation with style A behavior, objective support and support utilization; depersonalization has negative correlation with objective support and support utilization, P <0.01. Personal cope and decreased personal accomplishment have negative correlation with style A behavior and have positive correlation with other factors, among them, personal cope has positive correlation with active coping, objective support and subjective support; decreased personal accomplishment has positive correlation with active coping and support utilization, P <0.01. They have statistical significance.8. Multiple stepwise regression analysis by using general well-being and various sub-items of occupational stress and job burnout as dependent variables, and style A behavior, coping strategy, social support and normal data of nurses as independent variables. On the level ofα=0.05, five factors can jointly predict 18.07% variation of happiness, personal prediction power of support utilization is the strongest and its explanation power is 6.13%; six factors can jointly predict 20.17% variation of occupational role, personal prediction power of human relation is the strongest and its explanation power is 8.39%; six factors can jointly predict 24.05% variation of personal strain, personal prediction power of support utilization is the strongest and its explanation power is 11.38%; tow factors can jointly predict 10.78% variation of personal cope, personal prediction power of active coping is the strongest and its explanation power is 9.10%; five factors can jointly predict 21.58% variation of emotion exhaustion, personal prediction power of passive coping is the strongest and its explanation power is 10.86%; five factors can jointly predict 15.72% variation of depersonalization, personal prediction power of passive coping is the strongest and its explanation power is 6.71%; three factors can jointly predict 13.37% variation of decreased personal accomplishment, personal prediction power of active coping is the strongest and its explanation power is 9.33%.Conclusion1. Operating room nurses and other department nurses have different degrees of occupational stress. Operating room nurses experience adverse working environment, bear heavier body and mental stresses than normal persons, and don't decrease personal stress degree by improving rational emergency handling capacity and participating in entertainment and recreation actively.2. All of the operating room nurses and other department nurses have different degrees of job burnout. But percent of medium and heavy emotion exhaustion persons in operating room nurses is obviously higher than that in other departments, percent of heavy depersonalization persons in operating room nurses is obviously lower than that in other departments and percent of heavy decreased personal accomplishment persons in operating room nurses is obviously higher than that in other departments.3. General well-being has negative correlation with occupational role, personal strain, emotion exhaustion and depersonalization, and has positive correlation with personal cope and decreased personal accomplishment.4. General well-being has negative correlation with passive coping, and has positive correlation with other factors; occupational role, personal strain, emotion exhaustion and depersonalization have positive correlation with passive coping, and have negative correlation with other factors; personal cope and decreased personal accomplishment have negative correlation with style A behavior, and have positive correlation with other factors.5. The factor that has the largest influence on general well-being is support utilization; the factors that have the largest influence on occupational stress are human relation, support utilization and active coping; the factors that have the largest influence on job burnout are passive coping and active coping.
