Investigation Of Job Stress Among Nurses And The Evaluation Of The Intervention Effect Of Operating Room Nurses In Integrated Hospitals

Objective:To investigate and compare occupational pressure sources, coping styles, as well as physical and mental health conditions of nurses from ordinary departments and operating room, for seeking protective measures to reduce pressure who feel the stress is too large.Methods:Using random cluster sampling method,297nurses from general departments (internal medicine, surgical, obstetrics and gynecology, pediatrics, outpatient) and operating room in a first class hospital of Shandong province were investigated by questionnaires. The questionnaires included General Information Questionnaire, Nurse Job Stressors Scale (NJSS), Simplified Coping Style Questionnaire (SCSQ), Symptom Checklist (SCL-90). The survey results were analysed by descriptive analysis, single factor analysis, correlation analysis, multiple logistic regression analysis, etc.Results:Of310questionnaires,302(97.4%) questionnaires were returned, eventually198(98.3%) copies were valid. Results are as follows:1. Pressure source scores in each dimension were (from high to low):nursing workload and work time allocation (2.56±0.74), nursing profession and work (2.37±0.63), patient care (2.17±0.64), working environment and resources (1.99±0.78), management and interpersonal (1.80±0.68). It was "management and interpersonal" had the lowest score, which mainly manifested in the "low wages and other social welfare","too much workload","worry about mistakes and accidents in work","too much useless paperwork","low social position", and so on.2. There were no significant difference scores in coping style between nurses in general department and in operation room (P>0.05). In the face of pressure, nurses who take positive coping style were more than those who take negative coping style (P<0.05), while take negative coping style was positively related to the stress (P<0.01).3. Each of SCL-90factors was related to the negative coping style in a positive way (P<0.01).4. Theatre Nurses who presented compulsion, somatization, hostility, depression and interpersonal sensitivity in mental health aspects were higher than those who work in ordinary department and normal (P<0.05). Their mental health status were worse.5. Analysis of multiple linear regression between influential factors and job stress showed that negative coping style had significant impact on nurses’stress.6. By comparing the symptoms self-assessment scale score of nurses in the operating room before and after the intervention we can concluded that:after the intervention, all the factor scores of the symptoms self-assessment scale score of nurses in the operating room were lower than before and the difference was statistically significant (P<0.01); the ratio of the nurses with higher pressure in operating room was changed from62.3%to42.5%and the proportion of them in negative coping styles decreased from21.7%to5.5%.Furthemore,the differences of them before and after intervention were statistically significant (P<0.05).Conclusions:The degree of nurses’work pressure in this general hospital is in the medium. High pressure comes mainly from workload and time allocation, specialty nursing, patient care, etc. In the face of pressure, the majority of nurses take positive coping style, using negative coping style have greater impacts on physical and mental health. According to the changes, the nursing administrators should pay more attention to maintain the physical and mental health of operating room nurses.
