ObjectiveThrough clinical research Fusehexue decoction treatment of vitiligo(QI and Blood Unbalanced Syndrome),objective evaluate the clinical curative effect,estabhsh guidelines for the study of diagnostic therapy,generalize the common type of vitiligo(QI and Blood Unbalanced Syndrome)new effective treatment;Through the animal experiments and discussions to discover the mechanism the chinese medicine Fusehexue decoction treating vitiligo.MethodsIn the clinical part of the study,56 patients with vitiligo(QI and Blood Unbalanced Syndrome)were randomly divided into treatment group(28 cases)and positive control group(28 cases)with completely randomized random positive control drug observation method.In the treatment group,the oral drugs were Fusehexue decoction,while the control group was treated with Bai Ling Tablet.In addition,the treatment group and the control group were topical 0.1%pimecrolimus ointment.The observation time were:medication 0 days,medication 28 ± 3 days,medication 56 ± 3 days,the medication was cured 84 ± 3 days after follow-up.The lesion area,lesion area,target lesion white spot score,pigment score of target lesion and quantitative score of symptom grading were recorded to evaluate the effects of drugs on patients' condition.In the experimental research part,first part:The experimental guinea pigs were numbered by body weight,and were randomly divided into group A(blank control group),group B(vitiligo guinea pig model group),group C(low-dose Fusehexue decoction group),group D(high dose Fusehexue decoction group).Part 2:Group B,C and D,the guinea pig model of vitiligo was established by chemical decolorization with 5%hydrogen peroxide for 60 days.Part 3:C and D groups from the first 11 days,were given low-dose,high-dose Fusehexue decoction intervention,for 50 days.Part 4:The change of color and skin color of guinea pig model of vitiligo was observed from the naked eye every day,and the effect of Fusehexue decoction were observed.Part 5:After the end of the experiment,the skin tissue in the center of skin lesions of each group was taken,and the basal cells and spine cell melanin granules were observed by pathological section.The pathological basis of the Fusehexue decoction on the guinea pig model of vitiligo was observed.The concentration of TGF-?,IL-6,IL-17 and IL-23 were measured by ELISA,and the immune network of TH17/Treg-related cytokines was studied to reveal the immunological mechanism of vitiligo "QI and Blood imbalance" "theory of immune imbalance mechanism,Chinese medicine" to reconcile qi and blood "as the law of the Fusehexue decoction immune regulation.ResultsThe part of clinical research:After 3 months of treatment,Target lesion area indicators:the total effective rate was 85.2%in Fusehexue decoction group,and the total effective rate was 50%in BaiLing tablet group.There was significant difference between the two groups in statistical significance(P<0.05).The results showed that the target area of lesion area of the Fusehexue decoction group was better than that of BaiLing tablet group.Total lesion area indicators:the total effective rate was 85.2%in Fusehexue decoction group,and the total effective rate was 46.2%in BaiLing tablet group.There was significant difference between the two groups in statistical significance(P<0.05).The results showed that the total area of lesion area of the Fusehexue decoction group was better than that of BaiLing tablet group.After 3 months of treatment,the lesion score of target white spot,There was significant difference in Fusehexue decoction group before and after treatment(P<0.05).There was no significant difference in BaiLing tablet group before and after treatment(P>0.05).Compared with Bailing tablet group,the Fusehexue decoction group were better than those in Bailing tablet group(P<0.05).Target skin lesions pigment integral:There was significant difference in Fusehexue decoction group before and after treatment(P<0.05).There was no significant difference in BaiLing tablet group before and after treatment(P>0.05).Compared with Bailing tablet group,the Fusehexue decoction group were better than those in Bailing tablet group(P<0.05).In the improvement of TCM syndromes,There was no statistical difference between the two groups(P<0.05),indicating that the Fusehexue decoction group in the improvement of TCM syndrome and Bai Ling tablet group quite.In the part of experimental study:There was no significant difference in melanin distribution among the four groups(P>0.05).There was no significant difference in serum IL-6 between the four groups(P>0.05).In the IL-17 test,the IL-17 concentration in the blank group was similar to that in the high-dose Fusehexue decoction group(P>0.05),and the other three groups had statistical significance(P<0.05)Namely the blank control group = high-dose Fusehexue decoction group<low-dose Fusehexue decoction group<vitiligo guinea pig model group?In the IL-23 test,four groups of data by the normal test,blank control group and vitiligo guinea pig model group,high-dose Fusehexue decoction group statistical difference(P<0.05),There is no significant difference among blank control group,low-dose Fusehexue decoction group and high-dose Fusehexue decoction group(P>0.05).Namely low-dose Fusehexue decoction group = blank control group<vitiligo guinea pig model group = high-dose Fusehexue decoction group?In the TGF-?test,there were significant differences(P<0.05)between the vitiligo guinea pig model group and the low-dose Fusehexue decoction group,the high-dose Fusehexue decoction group and the blank control group.There was no significant difference between the low-dose Fusehexue decoction group and the high-dose Fusehexue decoction group(P>0.05).Namely vitiligo guinea pig model group>high-dose Fusehexue decoction group = low-dose Fusehexue decoction group>blank control group?ConclusionsFusehexue decoction in the target lesion area index,the total lesion area index,the target lesion white spot score,the target lesion pigment,integral is superior to Bailing tablet,Fusehexue decoction in the improvement of TCM symptoms and BaiLing tablet quite.No significant improvement was found in the melanin distribution of guinea pig skin.IL-6 in Guinea Pigs with Vitiligo had no significant change in the serum IL-6 level of guinea pigs with Fusehexue decoction.The levels of IL-17,IL-23 and TGF-? in guinea pigs with vitiligo were inhibited with Fusehexue decoction.
