ObjectiveThis study aimed to investigate the clinical efficacy of acupuncture points Shaoyang specific treatment of migraine, and with non-acupoint, in contrast to western medicine group. By objective evaluation Shaoyang specific acupuncture point in the treatment of migraine efficacy and safety, and thus the mechanism of acupuncture points Shaoyang specific treatment of migraine initial states that provide a basis for acupuncture treatment in clinical use.MethodsThis clinical study collected in September2012-2014January outpatient clinics of traditional Chinese medicine in Taiwan Yongxiang diagnosed migraine patients met the inclusion criteria and113patients. Study used a randomized controlled trial methods, migraine transfers allocated according to the ratio of1:1:1, Shaoyang specific point group37cases,38cases of non-through non-acupoint group,38cases of western medicine group. Western diagnostic criteria for migraine with reference to the2004International Headache Society (HIS) to develop the "International Classification of Headache Disorders," in migraine without aura, migraine aura diagnostic criteria. TCM diagnostic criterion-referenced1992national State Administration of TCM acute encephalopathy collaborative group discussions to develop a "head wind diagnosis and evaluation standard." Clinical evaluation are:general log entries (collected the patient’sage, gender, disease duration, history, etc.), headache observed indicators, WHO Quality of Life Scale, TCD (transcranial Doppler ultrasound) testing. Overall pain scores determine the efficacy of standard reference efficacy standards in1992the State Pharmaceutical Administration National Cooperative Group developed acute encephalopathy. Results This study were collected from113patients with migraine, in which case108cases qualified five cases the clinical trial process is not complete, remove and shedding5cases.By Shaoyang specific point group, non-acupoint group, western medicine group sex, age, duration, nature headache, headache class, family history, headache observed indicators, VAS pain scores, WHO Quality of Life Scale score, TCD (transcranial Doppler ultrasound) test were compared between groups, showing no significant difference between the three groups (P>0.05) on the base of data, suggesting that the three groups have a balanced general information.1. Comparison of observed indicators before and after treatment headachea. Three groups before and after treatment of headache episodes scoreBefore treatment, the three groups of headache episodes score was no significant difference, with balance. After two courses of treatment Shaoyang specific point group, the number of attacks of headache medicine treatment group score was significantly reduced compared with the non-acupoint group, the difference was statistically significant (P<0.05). At follow-up, Shaoyang headache episodes in patients with a specific point group score still on a low level, the other two groups score has gone up, there was significant difference (P<0.05) compared with the Shaoyang specific point group. Conclusions show that the end of the two courses, three different therapies for patients with migraine headache episodes score with varying degrees of improvement. In the follow-up found that after stopping treatment, Shaoyang specific points on therapy in controlling the number of seizures in patients with migraine is still a good effect, but also to reduce its number of episodes than just the end of treatment. Rather than by long-term efficacy of non-acupoint group and western medicine group less than ideal, and the control of migraine headaches in patients not as good as the number of attacks by specific acupoint group Shaoyang good effect.b. Comparison of the three groups before and after treatment headache duration scoreBefore treatment, the three groups of headache duration score was no significant difference, with balance. After two courses of treatment Shaoyang specific point group, duration of headache medicine in the treatment group had better improve, and non-acupoint group pairwise comparison, score reduction was statistically significant (P<0.05). At follow-up, Shaoyang specific point group, western medicine group scores despite the rebound, but compared with the non-acupoint group, the difference was statistically significant (P<0.05).Conclusions show that for patients with migraine headaches reduce the duration of respect, after two courses of treatment, the clinical efficacy of specific points Shaoyang group and western medicine treatment group were better, both quite effective; non-acupoint group for reduce migraine headaches in patients with duration of effect is not obvious. At follow-up, Shaoyang specific point group, western medicine group remains relatively low level, to improve the patient’s migraine headache duration forward better effect.c. Three groups associated with headache symptom scores before and after treatmentBefore treatment, the three groups of headache associated symptom score compared to the difference was not statistically significant, with a balance. After two courses of treatment, Shaoyang accompanied by a particular point group, western medicine group headache symptom score and non-acupoint group compared to both reduce the difference was statistically significant (P<0.05). At follow-up, three groups of headache associated symptoms score pairwise comparison, the difference was not statistically significant (P>0.05).Conclusions show that after two courses of treatment, Shaoyang specific point group, western medicine group can improve to some extent in patients with migraine headache accompanied by symptoms. Non-acupoint group accompanied by headache symptom score compared with before treatment, no significant improvement. In the long-term effect, three groups of headache accompanied by long-term symptom scores are not ideal.d. Headaches affect three groups before and after treatment comparison of living scoreBefore treatment, the three groups of headache affect the degree of life scores compared, the difference was not statistically significant, with a balance. After two courses, Shaoyang compared by a particular point group, western medicine group affect the degree of life scores and non-acupoint group, there are different degrees of decline between the two groups was statistically significant (P<0.05) differences. At follow-up, a significant difference (P <0.05) compared to the group through a specific point, the extent of the impact of western medicine group of life scores and non-acupoint group Shaoyang. Conclusions show that after two courses of treatment, the three groups can improve to some extent in patients with migraine headache affect the degree of life scores, rather than less efficacious than non-acupoint group Shaoyang specific point group, western medicine group is obvious. Shaoyang specific point group, western medicine group after four weeks treatment of headache affect the degree of life scores remained at a low level, and its long-term efficacy is acceptable, rather than by long-term efficacy of non-acupoint group as good curative effect.2. VAS pain scoresBefore treatment, the three groups of patients compared with VAS pain score difference was not statistically significant, with a balance. After two courses of treatment, Shaoyang specific point group, western medicine group VAS pain scores than the non-acupoint group decreased significantly (P<0.05). Follow-up of three groups of VAS pain score pairwise comparison, Shaoyang specific acupoint group ratings than non-acupoint group and western medicine group decrease (P<0.05).Conclusions show that specific point group Shaoyang, non-acupoint group, western medicine group which may be reduced in patients with migraine pain VAS score, Shaoyang specific point group, western medicine group in relieving migraine pain symptoms than non-respect non-acupoint group significantly. While western medicine to relieve pain better clinical efficacy, but not as good as its long-term analgesic efficacy of Shaoyang specific points significantly.3. Quality of life score before and after treatmentBefore treatment, the three groups of patients rated the quality of life compared with no significant difference, with balance. After two courses of treatment, Shaoyang specific point group, quality of life scores and western medicine treatment group compared to the non-acupoint group, has increased, the difference was statistically significant (P<0.05). Follow-up Shaoyang specific point group, western medicine group’s quality of life score remained at a high level, the difference between groups was statistically significant (P<0.05).Conclusions indicate, WHO Quality of Life Scale (WHOQOL-BREF) in patients with migraine may reflect the overall health and overall quality of life situation, we can see from the above statistics, Shaoyang specific point group, western medicine group after two courses of treatment the quality of life of patients had a better and obviously long-term efficacy. Non-acupoint group for improving the quality of life in patients with no obvious effect.4. TCD (transcranial Doppler ultrasound) before and after treatment to detect indicatorsa. Before three cerebral artery (ACA) to detect blood flow velocity before and after treatmentBefore treatment, the three groups of patients with anterior cerebral artery (ACA) flow velocity compared to no significant difference, with balance. After two courses, Shaoyang statistically significant group by a particular point, the anterior cerebral artery blood flow velocity Western medicine treatment group compared with non-acupoint group improved (P<0.05). Follow-up among the three groups was no significant difference (P>0.05).b. Three middle cerebral artery (MCA) blood velocity detected after treatmentBefore treatment, the three groups of patients with middle cerebral artery (MCA) blood flow velocity compared to no significant difference, with balance. After treatment, there was statistically significant after brain Shaoyang specific point group, western medicine group artery blood flow velocity than non-acupoint group differences (P<0.05). At follow-up, Shaoyang specific acupoint group of middle cerebral artery blood flow velocity than the non-acupoint group, reducing the western medicine group, the difference was statistically significant (P<0.05).c. After three cerebral artery (PCA) to detect blood flow velocity before and after treatmentBefore treatment, the three groups of patients with cerebral artery (PCA) flow velocity compared to no significant difference, with balance. After two courses, the three groups at follow-up after cerebral artery (PCA) blood flow velocity was no significant difference between groups (P>0.05) between two. Conclusions show that Shaoyang specific point group, western medicine group for improving migraine patients before cerebral artery, middle cerebral artery blood flow velocity have better clinical efficacy, from a numerical comparison, the two groups fairly. From the point of view long-term efficacy, Shaoyang specific points set in four weeks follow-up after treatment, blood flow velocity in the middle cerebral artery of patients remained at a good level, the role was significantly better than non-acupoint group and western medicine group. In contrast, non-acupoint group of migraine patients to improve poor brain artery blood flow velocity effect.5. The overall effect of the three groups after treatment comparison judgmentBy Ridit analysis, a significant difference (P>0.05) between the three groups after treatment, the overall effect, as follows:Shaoyang specific acupoints group recovered five cases, accounting for13.5%; markedly effective in14cases, accounting for40.5%; effective12cases,32.5%; ineffective in5cases, accounting for13.5%; total effective rate was86.5%. Non-acupoint group recovered one case,2.8%;8cases (22.2%); valid seven cases, accounting for19.4%; ineffective in20cases,55.6%; total effective rate of44.4%. Western medicine treatment group recovered six cases, accounting for17.1%; markedly effective in11cases, accounting for31.4%; effective in12cases, accounting for34.3%;6cases, accounting for17.2%; total effective rate was82.9%. Conclusions show that the overall effect of the three groups after treatment to a specific point group Shaoyang best western medicine group, followed by non-acupoint group not quite think. ConclusionThe overall efficacy of the treatment of migraine patients from the point of view of the judgment, Shaoyang specific points total effective rate was86.5%, non-acupoint group, the total effective rate was44.4%, western medicine group, the total effective rate was82.9%, Shaoyang after a particular point group, western medicine group for the treatment of migraine patients have better clinical efficacy, the clinical efficacy of the non-poor non-acupoint group. Shaoyang specific point group, western medicine group which may be reduced in patients with headache frequency, duration, relieve associated symptoms of migraine pain intensity, quality of life of patients have a better effect, both in the short-term efficacy assessment the difference is not obvious. Follow-up results showed that specific point group in Shaoyang number of indicators (such as headache episodes, VAS pain score, etc.) are better than the long-term efficacy of western medicine group, which showed that the town of Shaoyang specific point group of long-term pain, the effect is more obvious. Although non-acupoint group headache episodes score, affect the degree of life score, VAS pain score before treatment areas and a certain improvement, but the overall effect is not ideal, and the follow-up results were poor.In comparing patients with migraine cerebral artery blood flow measurement, the Shaoyang specific point group, western medicine group for improving the brains of patients with migraine before artery, middle cerebral artery blood flow velocity have better clinical efficacy, the two groups is quite. From the long-term efficacy, Shaoyang specific acupoint group effect was significantly better than non-acupoint group and western medicine group. Non-acupoint group of migraine patients for improving cerebral artery blood flow velocity had no significant effect. Acupuncture significant effect, no side effects, easy operation method, Shaoyang-specific acupuncture points acupuncture treatment of migraine reflects the "meridians before, attending clinics," the therapeutic effects of its significant clinical efficacy of acupuncture in treating migraine is provided certain basis.
