"Chronic Disease Management Starts With Me" Health Intervention Service Program

In the face of the fact that our population ageing is becoming increasingly severe and people's demand for health does not match the actual social services,the state has put forward the policy advocacy of "healthy China Action".The health service for the aged with chronic diseases is a beneficial breakthrough for social work to provide services for the aged.Social work to carry out health intervention services for the elderly with chronic diseases is of great significance to save medical resources and build a harmonious and stable social environment.This study is based on the "chronic disease management,starting with me" health intervention service program,under the guidance of the Maslow's hierarchy of needs,the dominance perspective theory and the activity theory,by using the methods of literature research,questionnaire survey and participant observation,the aim of health intervention service is to improve the self-health management ability of the elderly with chronic diseases,and to expand the scope of social work service.To understand the problems and needs of the elderly with chronic diseases in the preparatory phase of the project,to design the project programme in a demand-oriented manner,to recruit and train volunteers in the early stages of the project,to form a continuous professional volunteer service team,and to carry out health education and guidance services,in the further stage of the project,one-to-one health records will be established,targeted follow-up Intervention Services and health medical guidance services will be carried out,and related resources will be linked,the formation of targeted,continuous service model.In the feedback phase of the project summary,the effectiveness of the project evaluation,summary of the problems in the implementation of the project and make feasible recommendations.
