Study On The Implementation Experience Of Chronic Disease Self-management Project For The Elderly

This study interviewed the social workers who provided services for the “Self-Management Project for Chronic Diseases” and the main group of elderly people who received services,analyzed the project implementation experience,and explored the service effects and difficulties faced by the community in chronic disease self-management.And put forward the practical work and follow-up research on the community self-management project for chronic diseases in the elderly.The study found that in the process of social workers' implementation of the project,there are differences in the concept between the implementers and managers,the chronic disease patients can not be changed immediately,the service beneficiaries are single,the project funds are insufficient,and the service evaluation consumes a lot of time for social workers.This study believes that the cause of these phenomena comes from the government's purchase of service projects,which is restricted by the purpose and principles of the project.Therefore,the researcher proposes that the key to solving the problem is to actively play the leading role of social workers and improve the initiative of elderly patients with chronic diseases.In the past,the government was established and the organization was implemented.It should be changed to a social work organization based on the principle of social work,and the government approved the proposal to provide funds to overcome the above difficulties.
