Clinical Observation On Jianpi Xiaolan Recipe For The Treatment Of Insulin Resistance In Type 2 Diabetes With Spleen Deficiency And Phlegm-dampness Syndrom

Objective:This study explores the understanding of insulin resistance in type 2 diabetes mellitus in traditional Chinese medicine,and observes the curative effect of Jianpi Xiaodan Recipe on insulin resistance in type 2 diabetes mellitus with spleen deficiency and phlegm dampness syndrome.Methods:Patients who met the diagnostic criteria of diabetic insulin resistance were randomly divided into control group and experimental group by randomized controlled study.On the basis of lifestyle intervention,the control group was given metformin tablets orally.The experimental group was treated with Jianpi Xiaodan Decoction,decoction,one dose per day,for 3 months.The changes of clinical symptoms were evaluated before and after treatment.The changes of fasting and 2-hour postprandial blood glucose,glycosylated hemoglobin,fasting insulin level,insulin resistance index,GLP-1,GIP and other indicators were detected before and after treatment,as well as the adverse reactions and compliance of patients.All data were analyzed by SPSS26.0 statistical software.Results:(1)There were no abnormalities in the safety observation indexes of all subjects,and no obvious discomfort and adverse drug symptoms occurred during the study period,suggesting that the drug was safe in this study.(2)There was no difference in gender,age,course of disease,TCM syndrome score,blood glucose level(FPG,2hPG,HbA1c),fasting insulin level,insulin resistance index,gastrointestinal hormones(GIP,GLP-1)between the two groups before treatment(P>0.05),suggesting that the two groups were comparable.(3)Blood glucose,insulin and insulin resistance index:fasting blood glucose,2h postprandial blood glucose and glycated hemoglobin in test group and control group were significantly lower than before treatment,with statistical significance in intra-group comparison(P<0.01),but no significant difference in inter-group comparison of indicators(P>0.05);Fasting insulin level and insulin resistance index in test group and control group were significantly decreased compared with before treatment,with statistical significance in intra-group comparison(P<0.01),but no significant difference between groups(P>0.05).(4)Gastrointestinal hormone:the content of gastrointestinal hormone in test group and control group was significantly higher than before treatment,with statistical significance in intra-group comparison(P<0.01),and in inter-group comparison of various indicators,the experimental group was higher than the control group,with statistical significance(P<0.01).(5)Curative effect of TCM syndromes:the total effective rate of TCM syndromes in experimental group was 89.4%and 5 7.4%in control group,and the curative effect of TCM syndromes in experimental group was significantly better than that in control group(P<0.01);The TCM syndrome score was significantly lower than that of the control group(P<0.01).(6)Safety:No significant adverse reactions occurred in both groups during treatment.Conclusion:Jianpi Xiaodan Decoction has the same effect as metformin in reducing blood glucose,fasting insulin level and insulin resistance index,but has a stronger effect than metformin in improving the symptoms of spleen deficiency and phlegm dampness and increasing gastrointestinal hormones.
