ObjectiveTo observe the clinical effect of shuzheng granule on islet function of patients with type 2 diabetes mellitus with spleen deficiency and phlegm-dampness type,relevant indexes of blood glucose and islet function before and after treatment and TCM symptom scores were compared.From the perspective of "spleen deficiency and phlegm dampness",the pathogenesis of diabetes in traditional Chinese medicine and the connection between shuzheng granule and western medicine in improving islet function were discussed,providing more objective theoretical basis for its clinical application.Methods66 cases of type 2 diabetes mellitus patients with spleen-deficient phlegm-dampness syndrome meeting the inclusion criteria were randomly divided into experimental group and control group with 33 cases each,using the prospective single-blind randomized control method.Both groupps were given diabetes health education and basic treatment such as control of blood pressure and blood lipid.The control group was given intensive insulin treatment,and the experimental group was given additional shuzheng granules on the basis of intensive insulin treatment(twice a day for 1 bag).The course of treatment was 2 weeks.To observe AUCI3G0-120,AUCCP0-120,Homa-islet,Homa-IR,EISI,PI/I ratio and TCM symptom score before and after treatment,and to evaluate the clinical efficacy of shuzheng granule in improving islet function of spleen deficiency and phlegm-dampness type by using SPSS statistical software.Results1.Before treatment:Gender,age,course of disease,BMI,HbAlc,FBG,G30,G120,CP0,CP30,CP120,AUCBG0-120,AUCCP0-120,Homa-islet,Homa-IR,EISI,PI/I ratio and TCM symptom score were compared between the two groups,and the differences were not statistically significant(P>0.05).2.After treatment:In terms of blood glucose control,AUCBG0-120 in both groups was significantly lower than that before treatment(P<0.01),and the difference between the two groups after treatment was significant(P<0.01).In terms of improving islet function,AUCCP0-120,Homa-islet,EISI and PI/I ratio of the two groups were increased compared with that before treatment,and Homa-IR was decreased compared with that before treatment(P<0.01 or P<0.05).Comparison between the two groups showed that Homa-IR,EISI and PI/I ratio of the experimental group were significantly improved compared with the control group(P<0.01 or P<0.05).Although Homa-islet and AUCCP0-120 showed no significant difference(P>0.05),the difference of Homa-islet was significant compared with the control group,indicating that the improvement of islet function was more significant than that of the control group.In addition,from the c-peptide release curve image,the increase of c-peptide secretion curve in the experimental group was more significant than that in the control group.In terms of clinical symptoms of traditional Chinese medicine,the improvement of symptom score in experimental group was more significant(P<0.01),and the total effective rate was 96.7%,with statistically significant difference between groups(P<0.01).3.Safety aspects:No serious adverse drug reaction occurred to the subject during treatment.No obvious abnormality is found in the indexes such as blood routine,urine analysis,stool routine,liver function,kidney function and other indexes before and after treatment.ConclusionThe combination of shuzheng granule and insulin intensive treatment can better control blood glucose,improve insulin resistance more significantly,restore islet secretion function more significantly and better improve clinical symptoms related to spleen deficiency and phlegm-dampness type in T2DM patients.The comprehensive effect is significant,giving full play to the advantages of traditional Chinese medicine in the treatment of T2DM,which is worthy of clinical promotion and application.
