Abstract/Summary:
Objective: To observe the effect of cautionary moxibustion with governor’s vein combined with routine acupuncture on patients with posterior circulation ischemic vertigo(renal insufficiency type),and to discuss the clinical significance of cautionary moxibustion with governor’s vein combined with routine acupuncture in the treatment of posterior circulation ischemic vertigo(renal insufficiency type),so as to provide clinical basis for later treatment. Methods: According to the randomized and controlled clinical study method,60 patients with postcirculation ischemic vertigo of renal deficiency conforming to the diagnostic criteria of traditional Chinese and Western medicine were selected and randomly divided into cautery moxibustion acupuncture treatment group and conventional acupuncture control group, with 30 patients in each group. In the cauterizing moxibustion acupuncture group, the method of cauterizing moxibustion combined with conventional acupuncture was used, once a day, once for 30 minutes,5 times a week.On weekends, the cauterizing moxibustion was treated once every Monday and Friday, twice a week. The treatment of conventional acupuncture group is the same as that of cauterizing moxibustion acupuncture group.Subjective feeling and objective examination indexes were measured before and after treatment in both groups, and the curative effect was observed. One week for one course of treatment, four weeks in total. Before treatment and 4 weeks after treatment were followed up respectively, the scoring system of vertigo Rating Scale(DARS),vertigo Disorder Scale(DHI),quantification scale of vertigo symptoms were used, and vertigo symptom scores were evaluated by TCM vertigo symptom observation table and TCD in Guiding Principles for Clinical Research of New Chinese Medicines. To observe the changes of Vs and Vd of LVA,RVA and BA,so as to observe the clinical efficacy of the treatment of circulatory ischemic vertigo(renal insufficiency type)by cauterizing and moxibustion combined with routine acupuncture.Results:1.Curative effect: After treatment, the curative effect of the two groups was compared, the cautaxibustion moxibustion combined with conventional acupuncture group was better than the conventional acupuncture group, and the effect was obvious(P <0.05);The total effective rate was 93.33% in the treatment group and 83.33% in the control group.2.TCM syndrome score: After treatment, the scores of TCM syndrome in the two groups were decreased compared with those before and after treatment(P <0.05)through the comparison between the two groups before and after treatment(P=0.000(P <0.05),indicating significant difference. These results indicated that the group of cauching moxibustion combined with conventional acupuncture had better effect on improving vertigo symptoms than the group of conventional acupuncture.3.TCD results: The changes of Vs and Vd of LVA,RVA and BA in the cauterizing moxibustion and Du pulse combined with the conventional acupuncture group were significantly improved after treatment, and there were significant differences after treatment,P <0.05;These results indicated that the combination of cauterizing and moxibustion with routine acupuncture and routine acupuncture could improve the vertebrae-basilar artery blood flow velocity in patients with PCIV.At the end of treatment, the difference between the two groups was significant(P <0.05).The difference between the two groups was significant(P<0.05).These results indicate that the clinical application of cautionary moxibustion with acupuncture can significantly improve the vertebrobasilar artery blood flow velocity, and the effect is better than that of acupuncture alone.4.Comparison of DARS score and DHI score: The curative effect of the two groups was compared. After treatment,P<0.05,both DARS score and DHI score of the cautaxibustion acupuncture group and the conventional acupuncture group were decreased; The difference between the two groups and the difference between the groups before and after treatment P<0.05,indicating that the combination of cauching and moxibustion with routine acupuncture group decreased more significantly, and the therapeutic effect was better. Conclusion: Cauterizing and moxibustion with routine acupuncture therapy can effectively improve the vertigo symptoms of patients and improve the quality of life of patients. At the same time,it can improve the cerebral blood flow and effectively improve the vertebra-basilar artery blood supply, suggesting that in the acupuncture intervention of patients with vertigo caused by cerebral ischemia,we can use the cauloxibustion moxibustion with acupuncture manipulation,so as to have a better effect. It provides an accurate and effective method for the clinical treatment of postcirculation ischemic vertigo.
