ObjectiveThrough the clinical study of patients with posterior circulation ischemic vertigo, the clinical efficacy and safety of wheat grain moxibustion"seven acupoints of stroke" were observed, and its possible mechanism of action was discussed,so as to provide a reliable basis for the clinical application and promotion of wheat grain moxibustion"seven acupoints of stroke" in the treatment of posterior circulation ischemic vertigo.MethodsUsing the random number table method,60 patients with posterior circulation ischemic vertigo were randomly divided into 2 groups, with the ratio of treatment group and control group being 1:1,and 30 patients in each group. The control group was treated with conventional western medicine:0.9%sodium chloride injection 250ml+Sanqi Panax Notoginseng for Injection 400mg ivgtt qd,betastin mesylate tablets 6mg po tid, aspirin enteric-coated tablets 100mg po qd.and active control of hypertension, diabetes, hyperlipidemia and other risk factors. On the basis of the control group, the treatment group was treated with wheat grain moxibustion "seven acupoints of stroke":Baihui,Dazhui,Fengchi,Jianjing,Quchi,Jianshi,Zusanli,14 days for a course of treatment, and both groups were treated with a course of treatment.Recorded respectively in two groups of patients before and after treatment of vertigo dizziness assessment rating scale(DARS)score, dizziness handicap inventory(DHI)score, vertigo clinical syndrome evaluation scale scores,high-sensitivity C-reactive protein(hs-CRP)and homocysteine(Hcy)level changes, and trananial cerebral dpooler(TCD)to examine the collected mean blood flow velocity(Vm)and vascular pulse index(PI)of the bilateral vertebral artery(VA)and basilar artery(BA).SPSS23.0 statistical software was used for statistical analysis.Results1.Comparison of clinical efficacy:the total effective rate of the treatment group was 90.00%,and that of the control group was 76.67%.The difference between the two groups was statistically significant(P<0.05),indicating that the therapeutic effect of the treatment group was better than that of the control group.2.Comparison of DARS scores:after 14 days of treatment, the DARS scores of the treatment group and the control group decreased to different degrees compared with those before treatment, and the difference was protuberant statistically significance before and after treatment in the two groups(P<0.01).After treatment, the difference between the two groups was statistically significant(P<0.05).3.Comparison of DHI scores:after 14 days of treatment,DHI scores in both the treatment group and the control group decreased to different degrees compared with before treatment, and the difference was protuberant statistically significance before and after treatment in the two groups(P<0.01).After treatment, the difference between the two groups was protuberant statistically significant(P<0.01).4.Comparison of scores of the vertigo clinical syndrome evaluation scale:after 14 days of treatment, the scores of the vertigo clinical syndrome evaluation scale in the treatment group and the control group decreased to different degrees compared with those before treatment, and the difference was protuberant statistically significance before and after treatment in the two groups(P<0.01).After treatment, the difference between the two groups was statistically significant(P<0.05).5.Comparison of hs-CRP and Hcy levels:after 14 days of treatment,hs-CRP and Hcy levels in the treatment group and the control group decreased to a certain extent compared with before treatment, and the differences were protuberant statistically significance before and after treatment in the group(P<0.01).After treatment, the difference in hs-CRP level between the two groups was statistically significant(P<0.05).After treatment, there was no significant difference in Hcy level between the two groups(P>0.05).6.Comparison of TCD blood flow parameters:after 14 days of treatment, the mean blood flow velocity(Vm)and pulsation index(PI)of bilateral vertebral artery and basilar artery in the treatment group and the control group were improved to different degrees compared with those before treatment. The difference was protuberant statistically significant before and after treatment in the two groups(P<0.01);After treatment, the comparison of bilateral vertebral artery and basilar artery mean blood flow velocity between the two groups showed statistically significant differences(P<0.05),After treatment, there were protuberant statistically significant differences in bilateral vertebral artery and basilar artery pulse index between the two groups(P<0.01).Conclusion1.Wheat grain moxibustion "seven acupoints of stroke" combined with conventional western medicine treatment is superior to conventional western medicine treatment, with unique efficacy. The DARS score,DHI score, vertigo clinical syndrome evaluation scale score and TCD blood flow parameters of patients with posterior circulation ischemic vertigo can be clearly improved. The results showed that wheat grain moxibustion "seven acupoints of stroke" could improve the degree of vertigo and the syndrome of Chinese medicine and improve the quality of life.2.wheat grain moxibustionn "seven acupoints of stroke" can reduce the level of hs-CRP and fail to reduce the level of Hcy in patients with posterior circulation ischemic vertigo, which provides an idea for further research on the wheat grain moxibustion mechanism of posterior circulation ischemic vertigo3.No adverse reactions were found in "seven acupoints of stroke" of wheat grain moxibustion combined with conventional western medicine, which is worth promoting in clinical treatment.
