Research background:The current trend of negative population growth in China has accelerated the aging process,which will inevitably increase the medical burden and social pressure in China,among which,sarcopenia,a muscle degenerative disease,potentially affects the health conditions of the elderly population.Based on this,the China sarcopenia expert groups reached consensus and insisted on conducting a large sample,large area,and multi-site study on the cut-off value of sarcopenia in order to build a"health defense"for the elderly population,hoping to achieve the goal of"Healthy China2030"as early as possible.Research objective:Based on the Asian Working Group on Sarcopenia(AWGS2),the data of related indicators were measured,and statistical methods were used to select the research and judgment cut-off value of some indicators of the elderly in Wenzhou,and the second test was conducted through the new cut-off value.To evaluate the muscle health status of the elderly in Wenzhou and to provide evidence and reference for the research and diagnosis of sarcopenia.Research methods:492 community-dwelling elderly people from Wenzhou city were selected,and firstly,the basic AWGS in elderly people in Wenzhou city was collected by measuring human morphology,gait speed,grip strength,and assessment of the Enhancing SARC-F scale,using the consensus of the Asian sarcopenia expert group as a benchmark.Next,the muscle content of the elderly was calculated by regression equations using parameters such as height,weight,age,and limb dimensions,and the cut-off value of muscle content was determined using the latter 20%of the subject population as the cut-off point.Using the above muscle content cut-off value as a benchmark,the muscle content cut-off value for upper arm circumference and calf circumference were determined by drawing the receiver operating characteristic curve(ROC)for upper arm circumference and calf circumference.Thirdly,the cut-off value of grip strength were determined by plotting the receiver operating characteristic curves using the step speed(<1.0m/s)index as a benchmark.Finally,the muscle content cut-off value,grip strength cut-off value and gait speed(<1.0m/s)were pooled to determine the likelihood of sarcopenia in elderly people in Wenzhou,secondary tests as well as diagnostic and comparative analyses were performed in conjunction with the AWGS2 and the EWGSOP2.Research results:The initial screening by the Enhancing SARC-F scale revealed that 49 elderly males in Wenzhou city were at potential risk of sarcopenia,accounting for 31.2%;while 102 elderly females accounted for 30.4%.The regression equation from Chinese scholars was used to determine the muscle content cut-off value of 7.78kg/m~2(male)and 6.32 kg/m~2(female)for elderly people in Wenzhou city,and combined with ROC curves to determine the cut-off value of calf circumference and upper arm circumference of 33.8 cm and 25.8 cm for elderly men,while 31.3 cm and24.8 cm for elderly women.The new cut-off value of grip strength in elderly people in Wenzhou city was determined by using ROC curve with a new Asian consensus step speed<1.0 m/s as the standard,the new cut-off value was 28 kg for elderly men and14.7 kg for elderly women.The final diagnosis rate of sarcopenia in elderly people in Wenzhou city was 20.3%(male)and 19.4%(female),respectively.In the comparison with the new European consensus and the new Asian consensus,it was found that the new cut-off value of the diagnosis rate in this research was higher(19.7%),among which the diagnosis rates of the new Asian consensus and the new European consensus were 5.5%and 5.9%,respectively.Conclusions and Recommendations:1.The Enhancing SARC-F scale has a good diagnostic effect as a primary screening tool,although the positive prediction rate is low,but the negative prediction rate is maintained at a high level,it can be used as an effective primary screening tool in the early stage of sarcopenia.2.Although the muscle content cut-off value selected for this study were all higher than the two major consensus,the calf circumference cut-off value were lower than those initially set by the new Asian consensus.The grip strength cut-off value for elderly men was consistent with the AWGS2 while the cut-off value for elderly women was somewhat different than the AWGS2.3.The AUC values of calf circumference,upper arm circumference and grip strength in this cut-off value selection research were all greater,indicating that all three indexes had good diagnostic effects.4.Re-examination of the local population according to the new cut-off value set for the elderly population in Wenzhou found that the health status of the elderly population in Wenzhou was relatively good and the overall diagnose rate was within the normal range.5.It is suggested that subsequent studies should continue to include as many elderly groups as possible to reduce the error of cut-off value selection,and conduct age grouping based on large sample sizes to reflect the characteristics of sarcopenia as a degenerative disease.In addition,efforts should be made to integrate the knowledge of health promotion for the elderly into the physical education class,students should be taught simple measurement methods and age-appropriate training methods to improve the health status of the elderly population,thus improving the prevention and treatment effect of sarcopenia in Wenzhou.
