With continuous strengthening of health needs of people and deepening of medical reform process of public hospitals,public hospitals are facing rapid external-policy changes,fierce competition in the medical market,unified government department pricing,and high-cost staffing structures,which brings pressure on its operation.The performance appraisal system of tertiary public general hospitals implemented since 2019 also put forward specific requirements for public hospitals to transform from scaling up to improving quality and efficiency,from extensive management to fine management,and from investing in hospital development to improving employees' treatment.Therefore,it is necessary for public hospitals to implement modern management system,which contains establishment of a performance evaluation system based on workload evaluation,development in new technologies and new businesses,adjustment of disease,income and expenditure structure.The purpose is to improve the hospital's core competitiveness and to achieve improvement in the hospital's operation efficiency and benefits.This article takes the XD hospital,a tertiary public general hospital affiliated to central enterprise,as an example.By analyzing the status of performance appraisal system in public hospitals,this paper uses a RBRVS method to improve the performance appraisal plan for the physician grades that play a core role in hospitals,which is widely used in hospital medical expense payment in foreign countries.The principle of scheme design is to highlight the value of the work of employees and to scientifically evaluate the difficulty,risk and pressure of their work,and to stimulate the doctors' enthusiasm to improve the quality of medical services,which highlights the value-oriented role of performance appraisal system.According to the current situation of domestic pilot medical insurance payment reform,the designed performance appraisal plan has integrated key DRGs coefficient,so as to scientifically evaluate the difficulty of diagnosis and treatment of diseases.The program has also combined the dimension of the performance appraisal system of tertiary public general hospitals to design personalized key evaluation indexes for different departments,which conforms not only to the national policy guidance,but also to the development orientation of hospitals.This article has integrated DRGs with more advanced RBRVS tools to improve physician performance evaluation scheme,kept the non-profit characteristic of public hospitals,improved the technical level of the tertiary public general hospital,motivated the physician team work enthusiasm,achieved improvement in hospital internal management and external competitiveness,and laid a solid foundation for the development of hospitals health care reform and medical payment reform.At the same time,this article summarizes the problems in the promotion of RBRVS physician performance appraisal program,and puts forward targeted measures as well as a prospect for the future,which can provide a practical guidance for other enterprise hospitals in their development transition period.
