Objective: Resource Based Relative Value Scale(RBRVS)and Diagnosis Related Groups(DRGs)are the preferred performance management methods for hospital managers in recent years.This study,through the method of historical comparison,the evaluation model based on DRGs + RBRVS nursing performance after the application in cardiovascular internal medicine nurses work performance,work load,the effect of nursing quality and satisfaction,in order to improve the nurse's work performance,fully arouse the enthusiasm of nurses' work,and improve nursing quality and satisfaction,as the DRGs + RBRVS nursing performance provides some references for the popularization and application of the model.Methods: 1.Literature analysis: refer to relevant contents of nursing performance management model construction at home and abroad to understand the latest research status and trends of nursing performance management and DRGs+RBRVS at home and abroad.2.The application effect of DRGs+RBRVS nursing performance model in the department of Cardiology was compared by historical comparison;RBRVS group adopted the nursing performance model based on RBRVS from January to October 2018.The DRGs+RBRVS group adopted the nursing performance model based on DRGs+RBRVS from January to October 2019.The differences between RBRVS group and DRGs+RBRVS group in workload performance,nursing operation cost,workload,nursing quality and satisfaction evaluation were compared.Cluster sampling was used to investigate 121 medical staff and 5% of inpatients in the Department of Cardiology of a Grade A hospital in Xinjiang province in November 2018 and November 2019.IBM SPSS 21.0 statistical software was used to sort out and analyze the data,and descriptive statistics was used to analyze the general information of the study subjects.T test,non-parametric test and chi-square test were used to compare workload performance,nursing workload,key performance indicators,incidence of nursing adverse events and satisfaction score between THE RBRVS group and DRGs+RBRVS group.Results: 1.A total of 83 nurses and 38 doctors were enrolled,with two groups of medical staff as fixed objects;A total of 645 patients were investigated in the RBRVS group and 675 patients in the DRGs+RBRVS group.General data comparison showed no statistically significant difference.2.Workload performance of DRGs+RBRVS group(241368.00±9696.45)was higher than that of THE RBRVS group(195225.10±8275.79).Compared with the RBRVS group,the cost of nursing operation in DRGs+RBRVS group increased by 0.1-2.1%.3.The number of discharged patients in DRGs+RBRVS group was higher than that in RBRVS group,but the difference was not statistically significant(P > 0.05);Hours of intensive care,days of primary care and days of medium rescue were higher than those of the RBRVS group,and hours of specialized nursing operations such as ecg monitoring,telemetry ECG monitoring,days of small rescue and hours of non-invasive assisted ventilation were higher than those of the RBRVS group,with statistically significant differences(P < 0.001).4.The total score of nursing kpi in DRGs+RBRVS group(592.65±3.40)was higher than that in RBRVS group(590.23±2.76).The incidence of nursing adverse events was 0.59% in the RBRVS group and 0.37% in the DRGs+RBRVS group.5.The job satisfaction score(3.68±0.44)and performance satisfaction score(4.05±0.50)of nursing staff in DRGs+RBRVS group were higher than that of RBRVS group(3.51±0.49)and RBRVS group(3.67±0.53).Doctors' satisfaction with nurses(4.21±0.359)and inpatient experience and satisfaction(98.45±3.29)were higher than those of RBRVS group(4.20±0.143)and(96.85±4.92).Conclusion:(1)This model can effectively improve the workload performance of cardiology nurses,reduce the incidence of nursing adverse events,so as to improve patient satisfaction,and better play the regulatory role of performance management;(2)Two kinds of DRGs+RBRVS nursing performance schemes were adopted respectively in the general ward and the intensive care unit,which could conform to the characteristics of different wards and realize the refinement and objectivization of performance management;(3)The application of the nursing performance model based on DRGs+RBRVS is conducive to the objective and fair assessment of nurses' workload,and can also comprehensively consider the quality of nursing,which is in line with the requirements of the national nursing performance reform and DRGs pilot,and is worthy of promotion and reference.
