BackgroundICU nurses have a heavy workload, are in a high-stress environment for a long time, and are often exposed to secondary traumas such as trauma and pain in critically ill patients, resulting in the accumulation of empathy pressure and compassion fatigue.Compassion fatigue not only affects the physical and mental health of nurses, but also is one of the important reasons for the decline in the quality of patient care.At present, mindfulness has been used in nursing.Studies have shown that mindfulness-based intervention can effectively promote the physical and mental health of nurses, relieve their compassion fatigue and negative emotions, and improve work motivation.However,mindfulness-based stress reduction therapy can reduce ICU nurses’compassion fatigue. There is still a lack of scientific guidance for the application ofObjectiveTo understand the current situation and influencing factors of compassion fatigue among ICU nurses in my country, formulate an intervention plan for mindfulness-based stress reduction therapy, implement it,and evaluate its application effect,so as to provide a specific and actionable theoretical basis for alleviating compassion fatigue in ICU nurses.Methods1.Convenience sampling method was used to select 623 ICU nurses in 27 domestic tertiary hospitals as the research objects.General information questionnaire, professional quality of life scale, stress perception scale,five-factor mindfulness scale and Pittsburgh sleep quality index were used. The scale is a measurement tool, and a questionnaire survey is carried out on it.SPSS22.0 was used for statistical analysis of data, and independent samples t test,one-way ANOVA,Pearson correlation analysis and multiple linear regression were used to analyze the influencing factors of compassion fatigue among ICU nurses, with P<0.05 as the statistical significance.2.80 ICU nurses from a tertiary hospital in Henan Province were selected as the research objects and randomly divided into an intervention group and a control group. The intervention group received routine mental health education and 8-week mindfulness-based stress reduction therapy intervention, and the control group received routine psychological intervention.health education. Before and after the intervention, questionnaires were used to investigate the general data questionnaire, professional quality of life scale, stress perception scale,five-factor mindfulness scale and Pittsburgh sleep quality index scale of ICU nurses in the two groups to evaluate the application effect of the intervention program.SPSS22.0 was used for statistical analysis of data, and independent samples t-test,x~2 test or Fisher’s exact test were used to analyze the differences between the two groups before and after intervention, and P<0.05 was considered to be statistically significant.Result1.The compassion satisfaction score of ICU nurses was(31.01±6.93),the burnout score was(28.74±5.92),the secondary trauma score was(28.15±6.12),the compassion fatigue was at a severe level, the health status, weekly The number of night shifts, stress perception, sleep quality and mindfulness were the main influencing factors of compassion fatigue among ICU nurses(P<0.05).2.After mindfulness intervention, the compassion satisfaction score(36.92±6.69)of the intervention group was higher than that of the control group(28.10±3.48),the score of burnout(24.82±5.42)and the score of secondary trauma(23.52±4.21)were lower than those of the control group The scores of burnout(29.30±2.18)and secondary trauma(28.62±2.59)in the group were statistically significant(P<0.05),indicating that mindfulness-based stress reduction therapy can effectively improve the compassion fatigue of ICU nurses.3.After mindfulness intervention, the total score of stress perception(24.67±5.70),sense of loss of control(10.30±4.52)and tension(14.37±4.56)in the intervention group were lower than those in the control group(30.70±2.38)points,sense of loss of control(14.45±2.28)points,and sense of tension(16.25±2.48)points,the differences were statistically significant(P<0.05).4.After mindfulness intervention, the total score of mindfulness in the intervention group was(81.95±6.93),and the observation score was(14.37±2.24),description(17.05±2.90),perceived action(18.15±2.29),no judgment(16.35±2.00),no action(16.02±2.32),the scores of five dimensions were significantly higher than the total score of mindfulness in the control group(70.60±4.57)points,observation(10.80±2.16)points,description(14.35±1.74)points,conscious action(16.67±1.91)points,no judgment(15.22±2.29)points,no action(13.55±2.24)The difference was statistically significant(P<0.05),suggesting that mindfulness-based stress reduction therapy can improve the mindfulness of ICU nurses.5.After mindfulness intervention, the total score of sleep quality(4.72±2.29),sleep quality(0.60±0.54),time to fall asleep(0.92±0.82),sleep time(0.72±0.59),and sleep disorder(0.90±0.49)points,the scores of four dimensions were significantly lower than the control group’s total score of sleep quality(6.50±2.96),sleep quality(1.22±0.80),time to fall asleep(1.35±0.76),sleep time(1.25±0.70))score,and sleep disorder score(1.25±0.70),the difference was statistically significant(P<0.05).The rest of the three dimensions of sleep efficiency and hypnotic drug nursing daytime dysfunction score were lower than the control group, but the difference was not statistically significant The study showed that mindfulness-based stress reduction therapy had a certain effect on improving the sleep quality of ICU nurses(P>0.05).Conclusion1.The compassion fatigue of ICU nurses was at a severe level, and health status, weekly night shifts, stress perception, sleep quality and mindfulness were the main influencing factors of compassion fatigue in ICU nurses.2.The mindfulness intervention program can effectively improve the compassion fatigue of ICU nurses, reduce stress perception, improve the mindfulness of ICU nurses, and improve their sleep quality, thereby promoting the mental health of ICU nurses and improving the quality of nursing service.
