Objectives: The therapeutic efficacy of the combination of penetrating acupuncture and stagnant needle lifting method in the treatment of recalcitrant facial palsy with qi deficiency and blood stasis was observed using ordinary acupuncture as a control.The clinical effect of the combination of penetrating acupuncture and stagnant needle lifting method in the treatment of intractable facial paralysis was evaluated by the changes in the scores of H-B classification scale and Sunnybrook scale before and after treatment.This paper attempts to explore a more simple and efficient treatment scheme for the treatment of intractable facial paralysis.Method: A total of 63 patients who attended the outpatient and inpatient departments of the Department of Acupuncture and Moxibustion of Jingzhou Traditional Chinese Medicine Hospital between December 2019 and June 2021.They were diagnosed as intractable facial paralysis of qi deficiency and blood stasis type and met the inclusion criteria of this study.Randomly divided into32 cases in the treatment group and 31 cases in the control group according to the random number table,of which 3 cases fell off for their own reasons,2 cases in the treatment group and 1 case in the control group.Finally,a total of 60 cases with complete data were included in the statistics(30 cases in the treatment group and 30 cases in the control group).The treatment group was treated through acupuncture by penetrating needling combined with stagnant needle lifting(acupoints: Yangbai(GB14)-Yuyao(EX-HN4),Sizhukong(SJ23)and Taiyang(EX-NH5),Juliao(ST3)-Sibai(ST2),Xiaguan(ST7)-Quanliao(SI18),Jache(ST6)-Dicang(ST4),Qihai(RN6),Hegu(LI4),Zusanli(ST36)and Xuehai(SP10).The control group took the same points as above,but used ordinary acupuncture in the treatment.Treatment 1 time per day,5 times for a course of treatment,rest 2 days after the end of a course of treatment to enter the next course of treatment,a total of 8 courses of treatment.The changes of H-B grade,Sunnybrook and other curative effect observation indexes within and between the two groups were compared before and after treatment.The data recorded during the experiment were analyzed with SPSS 22.0 statistical software,and differences were considered statistically significant at P<0.05.Results:1.The differences in gender,disease location,disease duration and age comparison between the two groups before treatment were not statistically significant(P>0.05)and these two groups could be compared.2.Before treatment,the median H-B grade in the treatment group was 4(3,5)and that in the control group was 4(3,5);After treatment,the median of H-B grade in the treatment group was 2(2,3)and that in the control group was 3(2,3).The difference in H-B grade between the two groups before treatment was not statistically significant(P>0.05),and these two groups could be compared.There was a difference in H-B classification between the treatment groups after treatment(P<0.05),and the median of the treatment group was smaller than that of the control group;therefore,the efficacy of the treatment group was better than that of the control group.3.Before treatment,the median Sunnybrook score in the treatment group was 25.50(17.50,57.75),and that in the control group was 27.00(21.50,56.25);After treatment,the median Sunnybrook score was 84.50(80.00,90.50)in the treatment group and61.00(51.50,86.50)in the control group.The difference between the two groups before treatment was not statistically significant(P>0.05)and these two groups could be compared.There was a difference in Sunnybrook score between the two groups after treatment(P<0.05),and the median of the treatment group was significantly higher than that of the control group,so the efficacy of the treatment group was much better than another one.4.After treatment,6 patients in the treatment group were cured,16 were markedly effective,4 were effective and 4 were ineffective.The total effective rate was 87.0%;In the control group,4 cases were cured,6 cases were markedly effective,14 cases were effective and 6 cases were ineffective.The total effective rate was 80.0%.The difference between the two groups was statistically significant(P<0.05)by rank sum test analysis,and the treatment effect was better in the treatment group compared with the control one.Conclusion: Both ordinary acupuncture and penetrating acupuncture combined with stagnation needle lifting can effectively improve various indexes of facial nerve function in patients with intractable facial paralysis of qi deficiency and blood stasis type,promote the recovery of facial nerve function.However,compared with ordinary acupuncture,the effect of through acupuncture combined with stagnation needle lifting method is better,and the safety is guaranteed.It is suitable to be popularized in the clinical treatment of intractable facial paralysis of Qi,blood and blood stasis type.
