Clinical Observation Of TaiChi Tuina Rotating Manipulation In The Treatment Of Scapulohumeral Periarthritis

ObjectiveTo observe the clinical effect of Tai Chi Tuina Rotating Man ipulation in the treatment of Scapulohumeral Periarthritis,and compare the difference of curative effect between Tai Chi Tuina Rotating Manipulation and traditional shoulder Rotating Manipul ation,and observe the effect of Tai Chi Tuina on the GHD.MethodsSixty patients with scapulohumeral periarthritis recruited at from the outpatient department of Tuina/Rehabilitation Medicine Department of Hubei Provincial Hospital of Traditional Chinese Medicine from September 2020 to January 2021 were divided into the treatment group and the control group according to the random number table.Soft Tissue Manipulation of Tai Chi Tuina was taken as a basic treatment of both groups,and the treatment group was combined with Tai Chi Tuina Rotating Manipulation on the basic treatment,and the control group combined with traditional shoulder joint Rotating Manipulation on the basic treatment.Patients were treated once per day,20 minutes per time,5 times per course,2 courses in total,2 days off after the first treatment in both groups.VAS,Constant-Murley shoulder function score,range of motion(flexion,abextension,extension,and external rotation),and glenohumeral distance measured by musculoskeletal ultrasound were assessed before and after treatment in both groups,and adverse events during treatment were recorded.Finally,statistical analysis was made on the differences of the observation indexes in the two groups before and after treatment,as well as the differences between the treatment group and the control group before and after treatment.Result1.Basic information:After the treatment,2 cases were dropped from the treatment group and 1 case from the control group.The statistical sample size of the final treatment group was 28 cases,including 12 males and 16 females,with an average age of53.0±4.3 years and an average course of 7.8±3.2 months;and the statistical sample size of the control group was 29 cases,including12 males and 17 females,with an average age of 53.9±5.6 years and an average course of 8.2±3.0 months.2.Scale score before and after treatment:There was no statistically significant difference in VAS pain score and constant-Murley shoulder function score between the treatment group and the control group before treatment(P>0.05).At the end of treatment,the VAS scores of the two groups were lower than before(P<0.05),and the constant-Murley scores were both higher(P<0.05);Later,the VAS score of the treatment group was lower than that of the control group(P<0.05),while the constant-Murley score was significantly higher(P<0.05).3.Range of motion before and after treatment:Before treatment,there was no significant difference in the range of motion of the shoulder in each direction between the treatment group and the control group(P>0.05).After treatment,the range of motion in the four directions of flexion,abduction,extension,and external rotation in the treatment group was improved compared with that before treatment(P<0.05),while only the range of motion in the two directions of flexion and abduction in the control group was significantly higher than before(P<0.05).However,although the mobility in the two directions of extension and external rotation has a tendency to increase,there is no statistically significant significance(P>0.05).And at the end of the improvement in each direction of the treatment group was more obvious than that of the control group(P<0.05).4.Glenohumeral Distance:Among the 57 patients with unilateral scapulohumeral periarthritis,the glenohumeral distance on the affected side was significantly narrowed compared with that on the healthy side.After treatment,the glenohumeral distance was significantly improved in the treatment group and the control group(P<0.05),but remained narrow compared with that on the healthy side,and the glenohumeral distance was more improved in the treatment group than in the control group(P<0.05).Conclusion1.Compared with traditional shoulder Rotating Manipulation,Tai Chi Tuina Rotating Manipulation has a more significant clinical effect in the treatment of scapulohumeral periarthritis,and has advantages in alleviating pain,improving shoulder joint function,and improving Shoulder range of motion;Tai Chi Tuina Rotating Manipulation has the same effect on Shoulder range of motion in all directions of scapulohumeral periarthritis exceeding traditional shoulder Rotating Manipulation,especially in terms of external rotation and extension of the shoulder joint.2.The GHD of the affected side was significantly narrower than that of the healthy side in the patients with scapulohumeral periarthritis.After treatment,the GHD of the affected side was improved.Taiji massage can appropriately increase GHD,so as to improve the shoulder joint function.
