| Objective To observe the expression levels of human serum interleukin-1β(IL-1β)in hepatocelluar carcinoma(HCC)high-incidence family members and cancer-free family members,and to investigate the association between the human serum levels of interleukin 1β)(IL-1β)and the familial aggregation of HCC in HCC-epidemic region of Guangxi Province,China.Methods 120 subjects from families with high incidence of liver cancer were chosen as the research group,while 120 subjects from families without any malignant tumor were chosen as the control group,all of the study subjects were rural residents in the regions in Guangxi Province with high prevalence of HCC,and the control group and research group were paired in terms of age difference within 5 years,the same living environment,the same living habits,the same gender,the same nationality,and same HBV infection status,A total of 240 individuals without liver cancer were enrolled.In the families with high incidence of hepatocellular carcinoma,the mean±SD age of the members involved in the survey was(31.5±17.5)years.and there are total 62 male subjects,58 female subjects;51 subjects younger than or equal to 25 years old,69 subjects older than 25 years old;92 Zhuang populations,28 Yao populations;36 HBs Ag positive subjects,84 HBs Ag negative subjects.In families without any cancer,the mean±SD age of the control members involved in the survey was(32.2±17.4)years,there were total 62 male subjects,58 female subjects;51subjects younger than or equal to 25 years old,69 subjects older than 25 years old;92 Zhuang populations,28 Yao populations;36 HBs Ag positive subjects,84 HBs Ag negative subjects.In families with high incidence of hepatocellular carcinoma,there were 44 first-degree relatives of HCC probands(parents,children,brothers and sisters),23 second-degree relatives of HCC probands(grandparents,maternal grandparents,aunts and uncles),13 third-degree relatives of HCC probands(cousin brothers and sisters),40 fourth-degree relatives of HCC probands.There were 35 members whose familes had two or three patients with hepatocelluar carcinoma and 85 members whose families had four or more than four patients with hepatocelluar carcinoma.Enzyme-linked immunosorbent assay(ELISA)was used to identify serum interleukin-1β levels in the peripheral blood of all of all subjects.Results The overall expression level of serum cytokine IL-1β in peripheral blood of subjects from the research group and the control group was 21.495(15.275,36.040)pg/m L and 14.745(10.115,19.075)pg/m L,respectively.the results showed that the overall expression level of IL-1β in peripheral blood of subjects from the research group was significantly higher than those from the control group(Z=-4.777,P=0.000<0.05);and the stratification analysis was as following: the expression level of serum IL-1β of male members from the research group and the control group was 20.465(14.243,29.725)pg/m L and13.076(9.610,18.925)pg/m L,respectively.IL-1β expression levels of male members between the two groups showed statistically difference(Z=-2.731 and P=0.006<0.05);The expression level of serum IL-1β of female members from the research group and the control group was 23.700(17.063,42.709)pg/m L and 15.365(10.865,19.200)pg/m L,respectively.IL-1β expression levels of female members between the two groups showed statistically difference(Z=-3.922,P=0.000);The expression level of serum IL-1β of Zhuang population from the research group and the control group was 20.265(14.082,35.965)pg/m L and 12.530(9.360,17.133)pg/m L,respectively.the IL-1β expression levels of Zhuang population between the two groups showed statistically significant difference(Z=-4.287,P=0.000);The expression level of serum IL-1βof Yao population from the research group and the control group was 27.550(20.913,38.200)pg/m L and 19.700(17.028,24.538)pg/m L,respectively.the IL-1β expression levels of Yao population between the two groups were statistically different(Z=-2.072,P=0.038);The expression level of serum IL-1βof members who were younger than or equal to 25 years old coming from the research group and the control group was 21.550(16.524,41.070)pg/m L and15.580(11.170,18.800)pg/m L,respectively.the IL-1β expression levels of members who were younger than or equal to 25 years old between the two groups showed statistically significant difference(Z=-3.065,P=0.002);The expression level of serum IL-1β of members who were older than 25 years old coming from the research group and the control group was 21.100(14.163,31.229)pg/m L and 13.050(10.010,19.200)pg/m L,respectively.the IL-1βexpression levels of members who were older than 25 years old between the two groups showed statistically significant difference(Z=-3.686,P=0.000);The expression level of serum IL-1β of HBs Ag positive members from the research group and the control group was 17.750(13.988,20.808)pg/m L and 15.045(11.413,18.838)pg/m L,respectively.the IL-1β expression levels of HBs Ag positive members between the two groups were statistically significant different(Z=-2.074,P=0.038);The expression level of serum IL-1β of HBs Ag-negative members from the research group and the control group was 24.290(16.892,42.263)pg/m L and 13.885(9.435,19.488)pg/m L,respectively.the IL-1βexpression levels of HBs Ag negative members between the two groups were statistically significant different(Z=-4.887,P=0.000);The expression level of serum IL-1β of the first-degree relatives,the second-degree relatives,the third-degree relatives and the fourth-degree relatives of HCC patients in the HCC high-risk family was 20.683(14.001,30.568)pg/m L,20.575(15.563,40.955)pg/m L,21.550(20.380,52.100)pg/m L and 21.925(15.223,39.913)pg/m L,respectively.our research results exhibited that no statistical differences were observed among members with different blood kinship to the HCC patients from the HCC high-risk families(Z=1.648,P=0.642);the expression level of serum IL-1β of members from HCC high-risk families who had two or three patients and who had four or more than four patients with hepatocelluar carcinoma was 20.150(11.300,40.450)pg/m L,21.900(16.962,33.850)pg/m L,respectively.our research results exhibited that no statistical differences were observed among members with different numbers of probands in the HCC families(Z=-1.331,P=0.183),a total of 60 probands were chosen from 18 families with high incidence of liver cancer,including 46 male probands and 14 female probands,the prevalence rate of HCC among male members was76.667%,and the prevalence rate of HCC among female members was 23.333%,the prevalence rate of hepatocelluar carcinoma in the male subjects is much higher than that of female subjects.Conclusion(1)The high level of serum IL-1β is tightly associated with the familial aggregation of hepatocellular carcinoma in Guangxi.(2)The immune homeostasis of family members with HCC has been imbalanced under precancerous conditions instead of gradually occurring after hepatocyte carcinogenesis.(3)HBV infection is not the only factor of the familial clustering of hepatocellular carcinoma,and genetic susceptibility may play an important role in the familial clustering of hepatocellular carcinoma. |
