A Clinical Study On Auricular Acupuncture In The Treatment Of Breast Cancer-related Perimenopausal Syndrome

This article includes two literature reviews and one clinical research.The first review mainly summarizes the clinical application of auricular acupuncture in the diagnosis and treatment of malignant tumors in recent years,in order to provide more thinking and methods for the diagnosis and treatment of malignant tumors and related diseases.As a part of traditional Chinese acupuncture,auricular acupuncture is easy to be accepted by patients because of its advantages of safety,efficiency,convenience and economy.The ear are closely associated with the zang-fu organs,channels and collaterals and the body's various discomfort can be manifested in the ear through the changes in the color,shape,pain threshold and resistance of the ear points.The collection and analysis of these specific information is helpful for the early detection and diagnosis of gastric cancer,esophageal cancer,breast cancer and other malignant tumors,so as to create favorable conditions for improving the cure rate of cancer.Auricular acupuncture is a green and safe non-drug intervention measure,which also plays a positive role in the prevention and treatment of tumor-related diseases,especially have achieved good results in reducing chemotherapy related nausea and vomiting,cancer pain,anxiety and depression and other negative emotions,cancer fatigue,adverse reactions of endocrine therapy.It also has certain curative effect in assisting surgical anesthesia and postoperative recovery of tumor patients and improving respiratory symptoms.The second review mainly summarizes the current status of the treatment of breast cancer related perimenopausal syndrome in recent years,so as to provide reference and basis for the clinical treatment of breast cancer-related perimenopausal syndrome.Advances in breast cancer treatment have led to a continuous rise in survival rates,making it an urgent issue to improve the quality of life for breast cancer survivors.Breast cancer related perimenopausal syndrome is an important factor affecting the patients' quality of life.The clinical manifestations of breast cancer related perimenopausal syndrome are mainly vasomotor symptoms,psychoneurotic symptoms,musculoskeletal symptoms and urogenital symptoms.At present,the western medicine mainly adopts cognitive behavioral therapy and non-hormone drugs for symptomatic treatment.However,the limited curative effect,the tendency to produce new adverse reactions,the possibility of affecting the efficacy of tamoxifen and other adverse factors limit its clinical application.Traditional Chinese medicine and acupuncture have accumulated rich clinical experience in the treatment of breast cancer related perimenopausal syndrome,and they can intervene from multiple approaches and targets,with definite curative effect and fewer side effects.Especially,the auricular acupuncture therapy has a broad application prospect because of its advantages of simple operation and easy popularization.Objective:to observe the clinical efficacy of auricular acupuncture in the treatment of breast cancer related perimenopausal syndrome.Methods:patients with breast cancer related perimenopausal syndrome that met the inclusion criteria were collected from the Oncology department of China-Japan Friendship Hospital during December 1,2018 to June 1,2019,The enrolled patients were randomly divided into auricular acupuncture group and control group.The auricular acupuncture group was treated with auricular acupuncture combined with oral Chinese medicine,while the control group was treated with oral Chinese medicine only.To observe the changes of modified Kupperman score scale(KI),hot flash score(HFS),Pittsburgh sleep quality index(PSQI)and hospital anxiety and depression scale(HADS)before and after treatment.Results:a total of 34 patients in the auricular acupuncture group and 28 patients in the control group were included in the statistical analysis.There was no statistically significant difference between the two groups in the general condition of the subjects and the baseline level of KI,HFS,PSQI and HADS,so the two groups were comparable.After 4 weeks of treatment,KI,HFS,PSQI and anxiety and depression scores in the two groups were significantly lower than those at baseline,with statistically significant differences(P<0.05).After treatment,KI,HFS and PSQI levels in the auricular acupuncture group were significantly lower than those in the control group,with significant differences(P<0.05).There was no significant difference in the scores of anxiety and depression after treatment between the two groups(P>0.05).In terms of the score of 13 symptoms in the modified Kupperman scale,it was found that the score of the five symptoms of hot flashes,insomnia,palpitations,muscle and joint pain in the auricular acupuncture group was significantly lower than that in the control group,with statistically significant differences(P<0.05).The scores of sexual pain and urinary symptoms of two groups both had no significant difference in before and after treatment(P>0.05).In terms of efficacy analysis,the total effective rate of auricular acupuncture group at week 2,4 and 8 was significantly higher than that of the control group(58.8%vs 10.7%;91.1%vs 64.3%;79.4%vs 35.7%),the difference was statistically significant(P<0.01).Conclusion:(1)Both auricular acupuncture combined with traditional Chinese medicine and oral Chinese medicine alone can effectively improve the breast cancer related perimenopausal syndrome,but the overall efficacy of auricular acupuncture combined with oral Chinese medicine is significantly better than that of oral Chinese medicine alone,with the advantages of faster onset and more stable and lasting efficacy.(2)The efficacy of auricular acupuncture combined with traditional Chinese medicine is significantly better than that of oral Chinese medicine alone in improving the four symptoms of hot flash,sweating,sleep disorders,muscle and joint pain and palpitations in patients with breast cancer.The two interventions may be equally effective in improving the symptoms of excitable,anxiety and depression.In terms of improving urinary and reproductive symptoms,there may be no obvious effect of the two interventions.(3)During the treatment of auricular acupuncture combined with traditional Chinese medicine,there was only one patient with local pain intolerance after the treatment of auricular acupuncture,and no other or serious adverse reactions were found.So the application was relatively safe.
