Background:Breast cancer is the malignant tumor with the highest incidence among women in the world, and it is also the malignant tumor with the highest incidence among women in China. The incidence of breast cancer among women in China is increasing year by year.Among them, positive hormone receptor patients account for about 70%,and the number of patients is large.Endocrine therapy is an important treatment for this part of patients and has a long courseAbout 60%of patients will have different degrees of perimenopausal syndrome.Perimenopausal symptoms aggravate the patient's physical and mental burden and affect the patient's enthusiasm for treatment. Some patients cannot tolerate the treatment and then terminate the treatment, which affects their condition and prognosis.Clinically, there is no uniform standard for the treatment of perimenopausal syndromes related to breast cancer endocrine therapy.Modern medicine usually combines the symptoms of patients with the treatment of menopausal syndromes. The curative effect is still unclear and increases the economic and spiritual burden of patients. Therefore,it is necessary to find an active and effective treatment.External treatment of traditional Chinese medicine is a non-invasive treatment method advocated by the country at present.During COVID-19,the efficacy of traditional Chinese medicine has been further affirmed. Many early clinical studies have found that oral Chinese medicine or combined with acupuncture, magnetic beads auricular pressing therapies and other traditional Chinese medicine treatments are effective in improving hot flashes, sweating, and insomnia caused by endocrine treatment of breast cancer. However, some patients cannot be treated with traditional Chinese medicine or acupuncture because of disliking Chinese medicine and fearing needles.Magnetic beads auricular pressing therapies has the characteristics of simple operation, low price, easy to accept, and low side effects, and is widely used in many medical and surgical diseases in clinical practice. Objective:Objectively evaluate the clinical efficacy of magnetic beads auricular pressing therapies in improving the clinical efficacy of perimenopausal syndrome relating to breast cancer and provide safe and effective treatment methods for patients with breast cancer endocrine therapy-related perimenopausal syndrome. Methods:This study collected 60 patients with perimenopausal syndrome related to breast cancer endocrine therapy in Dongzhimen Hospital of Beijing University of Chinese Medicine and Haiyang People's Hospital of Shandong Province from July 2020 to December 2020.According to the principle of randomized control, patients were divided into treatment group(30 cases)and control group(30 cases)according to the principle of 1:1.The treatment group was given agnetic beads auricular pressing therapies, and the control group was a blank control.Auricular points are selected based on Sanjiao,Liver,Breast,Shenmen, and Jiaogan acupoints, and individualized addition and subtraction points based on the specific symptoms of the patient:subcortical points for hot flashes, heart points and Jianlv points for palpitations, subcortical points for insomnia, small occipital nerve points for anxiety and irritability, depression and excitement acupuncture, nerve acupuncture for pain kidney and inner ear acupoints for dizziness, kidney acupuncture for tinnitus and low back pain, auricular nerve acupuncture,Pi and Mouth acupoints for fatigue, lung and large intestine acupoints for those with skin ant-travel sensation, bladder and kidney acupoints for urinary system symptoms,Xiajiao and pelvic acupoints for sexual difficulties and gynecological symptoms.Replace the magnetic beads every 3 to 4 days, alternately press on both ears, and ask the patient to press 4-5 times a day by themselves, not less than 1 minute each time, better to feel soreness, numbness, and fever. The modified Kupperman scale and TCM symptom score were performed on the first day,the eighth day,and the fifteenth day of treatment. Finally,SPSS 20.0 was used for statistical analysis of the collected data.Results:1.There is no statistical difference in general conditions between treatment group and control group, and they are comparable between groups(P>0.05).2.Modified Kupperman score:On the 8th and 15th day after treatment, the KI value of the treatment group was lower than that of the control group, and there was no statistically significant difference in the KI score between the two groups on the 8th day(P=0.431>0.05),but there was statistically significant difference in the KI score between the two groups on the 15th day(P=0.001<0.05);the total effective rate of the two groups was higher than before, and the total effective rate of the treatment group on the 8th day higher than the control group(30%vs0%);the total efficiency of the treatment group was significantly higher than that of the control group(66.7%vs6.7%)on the 15th day,and the difference in curative effect between the two time periods was statistically significant(P<0.01).In terms of individual symptoms, the treatment group was effective in improving hot flashes, sweating, insomnia, paresthesia, irritability, dizziness, tinnitus, fatigue, backache, palpitations, skin ant movement, and sexual difficulties(P<0.05),among which hot flashes, sweating, insomnia, irritability, dizziness, tinnitus, back pain and fatigue were significantly improved(P<0.01),and the difference was statistically significant compared with the control group(P<0.05)3.TCM symptom scores:The scores of TCM syndrome in the treatment group were significantly lower than those in the control group on the 8th and 15th day after treatment, and there was no significant difference in TCM syndrome scores between the two groups on the 8th day(P=0.456>0.05),but there was significant difference in TCM syndrome scores between the two groups on the 15th day(P=0.002<0.01);the TCM symptom improvement index was significantly higher than before. The treatment group was higher than the control group(26.67%vs0%)on the 8th day after treatment, and the index of the treatment group was significantly higher than the control group(66.67%vs3.33%)on the 15th day,and the difference was statistically significant(P<0.05).In terms of individual symptoms, the treatment group had significant curative effects in hot flashes, sweating, insomnia, dizziness, tinnitus, upset, irritability, restlessness, chest pain, oliguria, and genital dryness, with statistical differences(P<0.05);in the control group, only upset, irritability, restlessness, and scores were lower than before, and there was no statistically significant difference in symptoms compared with before treatment(P>0.05).4.Efficacy in different age groups:On the 8th day of treatment, there was a certain efficacy in all age groups of the treatment group, but there was no statistically significant difference among all ages(P=0.597>0.05),while no efficacy was found in all ages of the control group. On the 15th day after receiving treatment, the effective rate of the treatment group increased compared with before, and there was no statistical difference in the effective rate among all groups(P=0.928>0.05).The effective rate of the perimenopausal group and postmenopausal group in the control group increased compared with before, but there was no statistical significance among all ages(P=0.561>0.05).On the 8th day of treatment, there was no statistically significant difference in the effective rate of patients of all ages between the two groups(P>0.05).On the 15th day of treatment, there was statistically significant difference in the efficacy of premenopausal and perimenopausal patients between the two groups(P=0.024<0.05,P=0.001<0.05),and there was no statistically significant difference in the postmenopausal group(P=0.173>0.05).Conclusions:Magnetic beads auricular pressing therapies can improve breast cancer endocrine therapy-related perimenopausal syndrome to a certain extent, especially in improving hot flashes, sweating, insomnia, irritability, dizziness, tinnitus, back pain and fatigue. It is worthy of clinical promotion.
