| Objective:To study the clinical effect of acupuncture combined with lactulose on functional constipation after lumbar internal fixation.Methods:From January 2019 to December 2019.50 patients with constipation af ter lumbar internal fixation who met the inclusion criteria and were admitted to the d epartment of orthopedics of Beijing Hospital of Traditional Chinese Medicine,Capital Medical University.They were randomly divided into treatment group and control grou p,25patients in each group.Treatment group:acupuncture combined with lactulose:m ain acupuncture points:Tianshu,Shangjuxu,Zhigou,Zhaohai.Matching acupoints:Hot secret plus Quchi and Hegu;Qi secret plus Zhongyuan and Taichong;cold secret plu s Guanyuan and Qihai;empty secret plus Zusanli and Qihai.Acupoint positioning and syndrome differentiation refer to the book "Acupuncture and Moxibustion" of the 12th Five Year Plan textbook of the Ministry of health of Chinese medicine colleges and u niversities.Among them,constipation,abdominal distention and pain,dry mouth and b ad breath,short and red urine,red tongue,yellow and dry fur,and slippery pulse are heat secrets;abdominal distention and pain,even with two flanks,thin and greasy fu r,and stringy pulse are gas secrets;abdominal cold pain,clear and long urine,light t ongue,white fur,and late pulse are cold secrets;although there is a sense of defecati on,it is empty secret to work hard in the toilet,or dry or not hard,thin tongue,and weak pulse.Specific operation:First,the patient is fully exposed to the parts to be nee dled,75%alcohol is used to disinfect the parts to be needled,and finally the disposa ble sterile acupuncture needle is used to operate according to the treatment method;o nce a day,keep the needle for 30 minutes,3 days as a course of treatment;at the sa me time,take lactulose oral solution(duphalac)for the first 30 ml,and then 25 ml e ach time,oral once a day,3 days as a course of treatment.Control group:lactulose o ral solution(duphalac)30ml each time,25ml each time after,oral,once a day,three days a course of treatment.Before treatment,one day,two days and three days after t reatment,PAC-SYM,PAC-QOL,Bristol stool classification,and the time from the first treatment to the first defecation were compared between the two groups.The therape utic effect was evaluated according to the diagnostic efficacy standard of traditional C hinese medicine.All data were analyzed under spss22.0 statistical software.Results:PAC-SYM:before treatment,there was no significant difference between the treatment group and the control group(P>0.05),PAC-SYM of the treatment gr oup and the control group decreased after 1,2 and 3 days of treatment,and the diffe rence was statistically significant(P<0.05)compared with before treatment;after 3 d ays of treatment,the difference was statistically significant(P<0.05)compared with the control group Meaning(P<0.05).Chinese version of constipation patients' quality of life scale(PAC-QOL score):before treatment,there was no significant difference between the two groups(P>0.05),PAC-QOL score of the patients in the treatment group was lower than that in the control group after 1,2 and 3 days of treatment,th e difference between the treatment group and the control group was statistically signifi cant(P<0.05);after 3 days of treatment,the difference between the treatment group and the control group was statistically significant(P<0.05)?Bristol defecation cl assification:there was no significant difference between the two groups before treatme nt(P>0.05).There was no significant difference between the treatment group and th e control group in the total effective rate after 3 days of treatment(P>0.05).The ti me from the first treatment to the first defecation had no significant difference betwee n the two groups before treatment(P>0.05),but there was significant difference bet ween the two groups after treatment(P<0.05).Evaluation of curative effect of tradit ional Chinese medicine:the total effective rate of the treatment group was 100%,and the total effective rate of the control group was 96%.There was no significant differ ence between the two groups(P>0.05).Conclusion:Acupuncture combined with lactulose and oral lactulose alone have c ertain curative effect on constipation after lumbar internal fixation.There is no signific ant difference in the total effective rate of Bristol stool classification and Traditional C hinese Medicine disease diagnosis standard between the two groups,but acupuncture c ombined with lactulose is better than lactulose alone in improving the symptoms,impr oving the quality of life and shortening the first defecation time after operation. |
