Purpose: The purpose of this study is to observe the clinical effect of medicinal moxibustion on primary dysmenorrhea with cold coagulation and blood stasis syndrome,and to explore the best dose-effect relationship of moxibustion,so as to provide clinical theoretical support for TCM diagnosis and treatment of primary dysmenorrhea.Methods: From July 2018 to September 2019,72 women who met the diagnostic criteria for primary dysmenorrhea due to cold coagulation and stasis syndrome were selected from the gynecology clinic of the affiliated hospital of Liaoning university of traditional Chinese medicine and those who were recruited through dormitory visits and online publicity.They were divided into observation group A and observation group B with 36 cases in each group.Group A was treated with medicinal moxibustion at Shenque and Guanyuan acupoints for 4 moxibustion volume and 7 days,while observation group B was treated with medicinal moxibustion at Shenque and Guanyuan acupoints for 8 moxibustion volume and 7 days.Both groups were treated continuously for 3 menstrual cycles.The changes of VAS score,CMSS scale score,dysmenorrhea symptom score,serum interleukin-10(IL-10)and tumor necrosis factor-?(TNF-?)before and after treatment were observed.Results:1.Total clinical efficacy:The total effective rates of group A and group B were 75.0% and91.7% respectively,compared with the two groups,there was no statistical significance(P>0.05).2.Comparison of curative effects between the two groups before and after treatment:After treatment,VAS score,CMSS scale score,dysmenorrhea symptom score,serum IL-10 and TNF-? levels in group A and group B were significantly lower than before treatment,it has statistical significance(P<0.05),this shows that medicinal moxibustion can effectively improve dysmenorrhea symptoms.3.Comparison of therapeutic effects between the two groups after treatment:There was no significant difference in VAS score,CMSS scale score,dysmenorrhea symptom score and TNF-? between group A and group B after treatment(P>0.05).The decrease of IL-10 in group B was significantly higher than that in group A(P<0.05).It shows that the two groups have the same curative effect in reducing VAS,CMSS,dysmenorrhea symptom score and TNF-?level.In terms of reducing IL-10 level,the curative effect of group B with large amount of moxibustion is better than group A with small amount of moxibustion.Conclusion:1.Medicinal moxibustion at Shenque and Guanyuan acupoints has definite therapeutic effect on primary dysmenorrhea due to cold coagulation and stasis syndrome,the observation indexes of the two groups after treatment were significantly lower than those before treatment.2.In terms of improving VAS,CMSS,dysmenorrhea symptom score and TNF-? level,the clinical efficacy of small moxibustion amount is equivalent to that of large moxibustion amount.3.In reducing IL-10 level,the curative effect of large amount of moxibustion is better than that of small amount of moxibustion.
